摘要
目的总结真空高负压引流系统在腹壁切口疝手术中的应用价值。方法选取2010年1月至2014年10月,山东省枣庄市立医院行腹壁切口疝无张力修补术52例患者,随机分为二组,高负压引流组26例,应用真空高负压引流系统引流;低负压引流组26例,应用传统低负压引流球囊引流。比较二组间的术后平均置管时间、术后平均住院时间、总引流量、皮瓣下积液发生率、切口愈合不良发生率、引流管细菌培养阳性率及术后围手术期的生活质量等指标。结果二组在术后平均置管时间、术后平均住院时间、皮瓣下积液发生率、切口愈合不良发生率等方面比较差异有统计学意义(t=5.400、4.025,χ2=5.65、4.127,P=0.000 1、0.000 2、0.017、0.042),总引流量、引流管细菌培养方面比较差异无统计学意义(t=0.837 3,χ2=0.754,P=0.406 4、0.385),高负压引流组患者在术后围手术期的生活质量有明显改善。结论真空高负压引流系统引流效果好,可减少术后并发症,促进切口愈合、减轻医护人员工作量,还可显著改善患者术后围手术期的生活质量,值得临床推广。
Objective To summarize the application experience of high vacuum wound drainage systems in ventral hernia repair. Methods 46 cases of ventral hernia who underwent tension-free repair in Zaozhuang City Hospital from January 2010 to October 2013 were collected. All patients were into two groups: high vacuum wound drainage group (23 eases) and traditional low vacuum balloon drainage group (23 cases). Further, the average postoperative drainage time, mean length of stay, total suction drainage, complications rate, bacterial positive rate of drainage tube and quality of life were analyzed and compared between the two groups. Results Comparing the average drainage time, mean length of stay, incidence of seroma and poor wound healing between the two groups. The results showed statistically significant differences (t=5. 400,4. 025,x^2 =5.65,4. 127,P =0. 000 1,0. 000 2,0. 017,0. 042). There were no significant differences in the total suction drainage and bacterial positive rate of drainage tube (t = 0. 837 3,x^2 =0. 754,P = 0. 406 4,0. 385 ). The patient using high vacuum wound drainage systems was associated with an improved postoperative quality of life. Conclusion The high vacuum wound drainage systems have shown a better effect, which can decrease postoperative complications, promote wound healing, reduce workloads, and can also significantly improve patients' postoperative quality of life. It is worth the clinical promotion.
