摘要
目的探讨血清降钙素原(PCT)检测在儿童感染性疾病中的应用价值。方法回顾新疆维吾尔自治区人民医院北院2010年1月至2012年1月收治的180例患感染性疾病儿童临床资料,对细菌培养情况、PCT、血清C反应蛋白(CRP)和白细胞计数(WBC)结果进行分析。依据细菌培养结果分组,细菌培养结果阳性即为细菌感染组(100例),细菌培养结果阴性为非细菌感染组(80例)。结果 180例患儿中,非细菌感染组血清PCT、CRP和WBC阳性率分别为5.0%(4/80)、72.5%(58/80)和7.5%(6/80),细菌感染组血清PCT、CRP和WBC阳性率分别为90.0%(90/100)、84.0%(84/100)和36.0%(36/100),显示细菌感染组血清PCT阳性率明显高于非细菌感染组,而CRP阳性率在细菌感染组和非细菌感染组中差异无统计学意义,细菌感染组WBC阳性率明显低于PCT阳性率。PCT阳性率与细菌感染程度呈正相关。结论作为区分细菌感染和非细菌感染的重要指标,患儿血清PCT检测对细菌感染性疾病的诊断明显优于CRP和WBC检测,PCT与患儿感染程度呈正相关,可作为疗效和预后评估指标。
Objective To evaluate application value of the serum proealcitonin (PCT) detection in children with infectious diseases. Methods Clinical data of 180 children with infectious diseases admitted in the North Wing of Xinjiang Autonomous Region People's Hospital from Jan. 2010 to Jan. 2012 were collected, including 100 cases with positive results of bacterial culture as the bacterial infection group, and 80 ease with negative results of bacterial culture as the non-infection group. Serum PCT, C-reactive protein (CRP) and white blood cell (WBC) detection results of the two groups were analyzed. Results PCT, CRP and WBC positive rates of all the cases were 5.0% (4/80) ,72.5% (58/80) and 7.5% (6/80) ,while in the bacterial infection group they were 90.0 % (90/100) ,84.0% (84/100) and 36.0% (36/100). The PCT positive rate of baeteria-infection group was signifieantly higher than the non-infection group ; there was no statistically signifieant difference of CRP positive rate between the two groups;in the bacterial infection group,the WBC positive rate was significantly lower than the PCT positive rate. All results showed that the PCT positive rate is positively correlated with the severity of infection. Conclusion As an important indicator for the differentiation of bacteria/infection and non-bacterial infection, the serum PCT is better than CRP and WBC,as it is positively correlated with the infection severity in children, therefore, the serum PCT can be used as an indicator to evaluate the treatment effect and prognaosis.
