ISOBAR TTL半坚强动态固定系统对腰椎固定节段椎间负荷的影响 被引量:1

摘要 目的 观察ISOBAR TTL动态固定系统对腰椎固定节段椎间负荷的影响.方法 选用6具新鲜人体腰椎标本,建立终板应力测试系统,选取L3椎体下软骨终板为测试应力部位,分别测量L3~L4运动节段软骨终板应力.L3~L4节段在椎间盘正常及摘除状态,分别进行TTL及坚强系统(SS)固定;依次使标本处于中立位、前屈、后伸、左弯、左旋,MTS858材料机施加轴向负荷,数据直接MTS机采集,由放大器放大后形成Excel文件.结果 无论在椎间盘正常或摘除组,固定节段L3~L4在直立、屈曲、伸展、侧屈及旋转等各种状态,TTL、SS系统固定状态下终板应力较无固定的正常标本明显减小,差异有统计学意义(P<0.05),各固定组间差异有统计意义(P<0.05),SS系统坚强固定下终板应力最低,而TTL系统固定下终板应力稍高于坚强组.结论 TTL半坚强动态固定系统能够起到明显缓解腰椎固定节段椎间载荷的作用,避免对固定节段的应力遮挡. Objective To study the effect of the ISOBAR TTL system on the fixed discal pressure.Methods Six fresh adult lumbar samples were tested.The L3-L4 segment,been studied in normal and disectomy state,was stabilized by TTL and 5 mm solid system relatively.Set up cartilage endplate stress test system,in the position of flexion 10°,extensionl0°,lateral bending10°,lateral rotation 10°,858 type MTS provided 400 N axial load,Then test and compare the stress on the endplate of the stabilized segment.Results In normal or disectomy state,the endplate stress (ES) deseased obviously after fixed by TTL and solid fixation system (SS) in every position,there existed significant variances between the fixation group and the normal group (P 〈 0.05),so did the TTL and SS group,the ES of the SS group was the lowest,while that of TTL group was a bit higher.Conclusion TTL system can alleviate the ES of the fixed segment obviously and avoid the stress block.
