实时超声造影在甲胎蛋白阴性肝脏中型和小型占位性病变中的诊断价值 被引量:5

Study on the Value of Real-time Contrast-enhanced Ultrasound in the Diagnosis of Middle and Small Focal Liver Lesions with Negative Alpha-fetoprotein

摘要 目的研究实时超声造影在甲胎蛋白(AFP)阴性肝脏中、小型占位性病变中的临床诊断价值。方法选择2012年1月至2013年1月深圳市龙华新区观澜医院收治的AFP阴性肝脏占位性病变患者91例,经术后病理诊断良性占位28例为良性组,恶性占位63例为恶性组,采用实时超声造影进行诊断,观察两组患者的时间-强度曲线参数,并比较实时超声造影、增强CT与病理结果的符合程度。结果恶性组患者的峰值时间、增强时间、上升斜率均显著升高,而始增强度、达峰强度、上升时间、半廓清时间均显著低于良性组,差异均有统计学意义(均P<0.01)。实时超声造影的灵敏度为78.57%,特异度为95.23%;增强CT的灵敏度为32.14%,特异度为79.36%。结论实时超声造影对AFP阴性肝脏中、小型占位性病变具有较高的临床诊断价值。 Objective To evaluate the value ot real-time contrast-enhanced ultrasound in the diagnosis of middle and small focal liver lesions with negative alpha-fetoprotein (AFP).Methods A total of 91 patients with middle and small focal liver lesions with negative alpha-fetoprotein admitted in Guanlan People's Hospital of Longhua New District from Jan.2012 to Jan.2013 were selected.After postoperative pathological diagnosis,28 patients with benign lesions were regarded as benign group,and the other 63 patients with malignant lesions were regarded as the malignant group.The real-time contrast-enhanced ultrasound examination were conducted in the two groups for diagnosis,the TIC parameters of the two groups were observed,and the coincidence degree of real-time ultrasound and enhanced CT with the pathological results were compared.Results The peak intensity,enhance strength,rising slope of the malignant group significantly rose,and the beginning enhanced degree,peak intensity,rising time,half-clearance time of the malignant group was significantly lower than the benign group,the difference was statistically significant (P 〈 0.01).Sensitivity of real-time contrast-enhanced ultrasound was 78.57%,specificity was 95.23% ; sensitivity and specificity of enhanced CT was 32.14%,79.36%.Conclusion Real-time contrast-enhanced ultrasound has great values in the diagnosis of middle and small focal liver lesions with negative alpha-fetoprotein.
