摘要
目的比较他汀类药物治疗动脉粥样硬化的疗效。方法将2012年3月至2013年3月嘉鱼县人民医院收治的84例动脉粥样硬化患者按照随机数字表法分成试验组和对照组,各42例。试验组采用瑞舒伐他汀治疗(每次10 mg,每日1次,服用24周),对照组采用阿托伐他汀治疗(每次10 mg,每日1次,服用24周)。观察比较两组患者的血脂水平、颈动脉内中膜厚度、斑块大小、炎性因子水平及血管内皮功能。结果治疗后试验组多项指标均低于对照组,分别是血清总胆固醇[(3.1±0.6)mmol/L比(4.2±0.6)mmol/L]、三酰甘油[(1.5±0.4)mmol/L比(1.7±0.4)mmol/L]、低密度脂蛋白胆胆固醇[(1.7±0.4)mmol/L比(2.5±0.4)mmol/L](P<0.05);试验组治疗后的高密度脂蛋白胆胆固醇[(2.2±0.6)mmol/L比(1.8±0.6)mmol/L]显著高于对照组(P<0.05);试验组治疗后的颈动脉内中膜厚度[(1.05±0.11)mm比(1.21±0.14)mm]、横切面最大面积[(13.02±2.48)mm比(15.26±2.37)mm]、纵切面最大厚度[(1.92±0.15)mm比(2.03±0.31)mm]、斑块面积[(19.70±8.25)mm2比(22.85±7.32)mm2]均显著小于对照组(P<0.05);试验组治疗后的血清高敏C反应蛋白[(1.55±0.23)mg/L比(2.21±0.45)mg/L]显著低于对照组,脂联素[(24.38±4.05)μg/L比(21.06±3.88)μg/L]显著高于对照组;试验组治疗后的血清一氧化氮[(22.14±5.36)μmol/L比(18.06±3.22)μmol/L]、血管内皮素1[(18.70±4.25)ng/L比(21.03±3.32)ng/L]均优于对照组(P<0.05)。结论瑞舒伐他汀能够明显改善动脉粥样硬化患者的血脂水平,减轻炎症发生,改善血管内皮功能,药理作用更强。
Objective To investigate the efficacy of different statins in the treatment of atherosclerosis.Methods A total of 84 cases of atherosclerosis admitted in Jiayu County People' s Hospital from Mar.2012 to Mar.2013 were randomly divided into two groups,the experimental group(n =42) and the control group(n =42).Patients in the experimental group were treated with rosuvastatin calcium (10 m at a time,once per day,for 24 weeks),and patients in the control group were treated with atorvastatin calcium (10 mg at a time,once per day,for 24 weeks).After treatment,the changes of lipid levels,carotid artery intimamedia thickness,plaque size,inflammatory cytokines level and vascular endothelial function of the two groups were compared.Results After treatment,the serum total cholesterol level[(3.1 ± 0.6) mmol/L vs (4.2 ± 0.6) mmol/L],triglycerides level [(1.5 ± 0.4) mmol/L vs (1.7 ± 0.4) mmol/L],low-density lipoprotein cholesterol level[(1.7 t0.4) mmol/L vs (2.5 s0.4) mmol/L] of the experimental group were significantly lower than the control group (P 〈 0.05) ; the high-density lipoprotein cholesterol level [(2.2 ± 0.6) mmol/L vs (1.8 ± 0.6) mmol/L] of the experimental group was significantly higher than the control group (P 〈 0.05) ; the carotid artery intima-media thickness [(1.05 ± 0.11)mm vs (1.21 ± 0.14) mm],the largest cross-sectional area [(13.02 ±2.48) mm vs (15.26 ±2.37) mm],the maximum thickness of longitudinal section[(1.92 ± 0.15) mm vs (2.03 ± 0.31) mm],plaque area [(19.70 ± 8.25) mm2 vs (22.85 ± 7.32) mm2] of the experimental group were significantly lower than the control group (P 〈 0.05) ; the high-sensitivity C-reactive protein level [(1.55 ± 0.23) mg/L vs (2.21 ± 0.45) mg/L] of the experimental group was significantly lower than the control group,adiponectin level [(24.38 ± 4.05) μg/L vs (21.06 ±3.88) μg/L] of the experimental group was significantly higher than the control group(P 〈 0.05) ; serum nitric oxide level [(22.14 ± 5.36) μmol/L vs (18.06 ± 3.22) μmol/L],endothelin 1 level [(18.70 ±4.25) ng/L vs (21.03 ±3.32) ng/L] of the experimental group were all better than the control group(P 〈 0.05).Conclusion Rosuvastatin can significantly improve lipid levels in patients with atherosclerosis,reduce inflammation,improve endothelial function,with stronger pharmacological effects.
