锁定型后足融合髓内钉行踝关节融合的早期疗效 被引量:2

摘要 目的 探讨应用锁定型后足融合髓内钉(HFN)行踝关节融合治疗踝关节终末疾患的早期疗效.方法 回顾性分析2012年1月至2014年1月采用锁定型HFN行踝关节融合治疗踝关节终未疾患并获半年以上随访患者15例,男6例,女9例;年龄32 ~ 72岁,平均61岁;其中踝关节骨关节炎4例,创伤性关节炎6例,类风湿关节炎5例.采用Kofoed踝关节评分、美国骨科足踝外科协会(AOFAS)踝-后足评分、Maryland足部评分及视觉模拟评分(VAS)评价术前及术后随访时足踝关节功能及疼痛情况.结果 15例患者术后获6~ 24个月(平均15个月)随访.末次随访时踝关节均已融合,13例患者对手术非常满意,1例患者对手术满意,1例患者于术后6个月时出现踝部疼痛影响行走,经去除内固定物后症状缓解,对手术感到满意.所有患者术后1、3、6个月的Kofoed评分、AOFAS踝-后足评分、Mary-land评分、VAS评分均较术前改善,差异有统计学意义(P<0.05,表1),而术后不同时间点间比较差异均无统计意义(P>0.05).结论 应用锁定型HFN行踝关节融合治疗踝关节终末疾患早期可减轻疼痛、矫正畸形、重建踝部功能.
