摘要
[目的]观察艾叶散熏洗联合髋关节功能恢复训练对老年股骨转子间骨折PFNA内固定术后髋关节功能影响。[方法]使用随机平行对照方法,将61例住院患者按病志号抽签方法简单随机分两组。对照组30例常规髋关节功能恢复训练。治疗组31例术后第七天使用艾叶散（艾叶30g,花椒、肉桂、透骨草各20g,当归、红花、防风、麻黄、威灵仙各15g,荆芥、桂枝、独活、海桐皮、白芷各10g）,在1000m L清水中煮沸30min,将患肢洗净,熏洗盆上方安放水管状连接器,便于聚集蒸汽熏洗髋关节,用浴巾围住患者肢体,使药液蒸汽可充分均匀的熏蒸化患处,20～30min/次,3次/d;常规髋关节功能恢复训练同对照组。连续治疗3周为1疗程。观测临床症状、患髋活动情况、X线检查、不良反应。连续治疗4疗程,随访12个月,判定疗效。[结果]治疗组优16例,良13例,可1例,差1例,总有效率93.50%。对照组优8例,良10例,可9例,差3例,总有效率60.00%。治疗组疗效优于对照组（P〈0.05）。[结论]艾叶散熏洗联合髋关节功能恢复训练对老年股骨转子间骨折PFNA内固定术后髋关节功能恢复,疗效满意,无副作用,值得推广。
[ Objective ] Observation of Ai Yesan Fumigation combined with the recovery of hip joint function training on the effect of PFNA internal fixation of hip joint function afterintertrochanteric fractures in the elderly. [ Methods] Using randomized controlled methods, 61 eases of hospitalized patientsaccording to disease by the method of simple random divided into two groups. A control group of 30 cases of normal hip function recovery training.The treatment group of 31 cases in seventh days after operation the use of Ai Yesan（ leaf 30g, pepper, cinnamon, Speranskia, grass 20g, angelica, safflower, windproof, ephedra, Clematis each 15g, Herba sehizonepetae, Ramulus Cinnamomi, Radix Angelicae Pubeseentis, erythrina bark, angelica each 10g ）, The boiling water in the 1000mL 30min, and then clean the patient＇s limb, Fumigation basin is installed above water pipe connector, easy togather steam fumigating hip, surrounded the limb with a towel, so that the liquid medicine steam can be sufficiently and uniformly fumigation affected part, 20～ 30min/3times everyday; conventional hip to resume training with the control group Festival function. Continuous treatment for 3 weeks as 1 course of treatment. Observation of clinical symptoms, patients with hip joint activities, X-ray examination, adverse reaction. Continuous treatment of 4 courses of treatment, follow-up of 12 months, curative effect.[ Results ] ：The treatment group 16 cases of excellent, good in 13 cases, general in 1 cases, poor in 1 cases, the total efficiency of 93.50%. The control group 8 eases of excellent, good in 10 cases, general in 9 eases, poor in 3 cases, the total efficiency of 60%. The treatment group was better than control group （ P 0.05 ） . [ Conclusion ] Ai Yesan fumigation combined with the recovery of hip joint function training on the recovery of hip joint function after PFNA, fixation of intertrochanteric fractures in the elderly with satisfied effect, no side effect, is worth promoting.
