摘要
目的:探讨小剂量红霉素对特发性肺间质纤维化血清转化生长因子β(TGF-β)、白介素13(IL-13)的影响。方法:将39例特发性肺纤维化患者随机分为常规治疗组(A组)和小剂量红霉素治疗组(B组),采用双抗体夹心ABC-ELISA法对肺间质纤维化两组及健康对照组(C组)血清TGF-β、IL-13进行测定。结果:A、B两组血清TGF-β及IL-13水平均明显高于C组(P<0.05,P<0.05)。治疗后B组动脉血氧饱和度明显高于A组,差异有显著性(P<0.05);B组血清TGF-β、IL-13水平均低于A组,但差异无显著性(P>0.05)。血清TGF-β、IL-13水平与血氧分压、血氧饱和度、肺泡-动脉氧分压差无直线相关关系。结论:细胞因子的紊乱与特发性肺间质纤维化有密切关系,红霉素可影响特发性肺间质纤维化的炎症反应,改善缺氧。
Objective:To investigate the effect of small-dose erythromycin on serum TGF-β and IL-13 in patients with idiopathic pulmonary fibrosis. Methods: Thirty-nine patients with idiopathic pulmonary fibrosis were randomly divided into two groups, conventional glucocorticoid treatment was used in 20 cases (Group A) and small-dose erythromycin was administrated in another 19 cases (Group B). Serum transforming growth factor-β (TGF-β) and interleukin-13 (IL-13) were detected in Group A and B and healthy controls (Group C) by double antibody sandwich ABC-ELISA. Results: Serum TGF-β and IL-13 levels of both Group A and Group B were significantly higher than those of control (P〈0.001, P〈0.05, respectively). The average level of SaO2 in Group B was significantly higher than that in Group A after treatment of small-dose erythromycin. No linear correlation was found between serum level of TGF-β and IL-13 and PO2, SaO2, and AaDO2. Conclusion: Disorder of cytokines is associated with idiopathic pulmonary fibrosis, and small-dose erythromycin treatment could mitigate inflammation in idiopathic pulmonary fibrosis.
