摘要
目的探讨胃肠道肿瘤术后手术部位感染的影响因素。方法采用病例对照研究方法,对7 209例胃肠道肿瘤手术病人的术前状态、肿瘤病理学因素和手术方法等指标进行Logistic回归分析。结果单因素分析显示,心脏病、糖尿病、营养不良、ASA评分、肿瘤部位、肿瘤TNM分期、联合脏器切除、术中输血、手术时间及手术失血量与胃肠道肿瘤术后手术部位感染有关。多因素分析显示,共有7个变量为胃肠道肿瘤术后手术部位感染的独立危险因素,分别为肿瘤部位(直肠、小肠、结肠)、ASA评分(Ⅱ级、Ⅲ级、Ⅳ级)、心脏病、营养不良、糖尿病、手术时间和手术失血量。结论应针对胃肠道肿瘤术后手术部位感染发生的上述主要影响因素进行必要的临床干预。
Objective To investigate the main influencing factors of surgical site infections (SSI) following resection of tumors in gastrointestinal system. Methods Case-control study was applied to investigate 7 209 patients with gastrointestinal tumors in regard to their preoperative conditions, oncopathologieal factors and operation methods, logistic regressive analysis was done for the above items. Results Univariate analysis showed that the factors related to SSI were as follows: cardiovascular diseases, diabetes mellitus, malnutrition, ASA score, tumor location, TNM stage, combined organ resection, intraoperative blood transfusion, operation time, and operation blood loss. Multivariate analysis showed that a total of seven variables were independent risk factors for SSI, there were, respectively, tumor location (rectum, small intestine and colon), ASA score ( Ⅱ,Ⅲ,Ⅳ), cardiovascular diseases, malnutrition, diabetes mellitus, operation time and intraoperative blood loss. Conclusion Necessary clinical interventions should be carried out aiming at the above mentioned factors affecting surgical site infections following gastrointestinal tumor resections.
