同型半胱氨酸与原发性高血压患者肾功能关系的研究 被引量:3

Association of homocysteine with renal function patients in primary hypertension patients

摘要 目的探讨同型半胱氨酸(Hcy)与原发性高血压患者肾功能关系。方法纳入2012年我院原发性高血压患者共814例。据Hcy值分为H型高血压(Hcy≥10μmol/L)与非H型高血压(Hcy<10μmol/L)组,分别比较两组患者的肾功能的差异,采用线性回归模型分析Hcy与肾功能相关指标间的关系。结果 H型高血压患者肌酐[(105.9±47.1)μmol/L vs.(88.6±55.9)μmol/L,P<0.001]、尿酸[(360.2±106.2)mmol/L vs.(309.4±83.8)mmol/L,P<0.001]显著高于非H型高血压组,e GFR[(57.1±5.0)ml·min-1·(1.73 m2)-1 vs.(68.8±23.4)ml·min-1·(1.73 m2)-1,P<0.001]显著低于非H型高血压组。回归分析发现高血压患者中Hcy水平与肌酐(β=1.542,P<0.001)、尿酸(β=2.845,P<0.001)呈正相关,与e GFR呈负相关(β=-0.726,P<0.001)。H型高血压患者中Hcy水平与肌酐(β=1.028,P<0.001)、尿酸(β=1.661,P<0.001)呈正相关,与e GFR呈负相关(β=-0.482,P<0.001)。结论 Hcy与原发性高血压患者肾功能相关,尤其是H型高血压患者。 Objective To explore the association of homocysteine(Hcy) with renal function patients in primary hypertension. Methods 814 cases with primary hypertension were included in 2012. They were divided into H-type hypertension group (Hcy≥10 μmol/L) and non H-type hypertension group (Hcy〈10 μmol/L) according to the Hey level. The differences in renal function between groups were compared. The linear regression model was used to analyze the relationship between Hey and parameters of renal function. Results The mean age of H-type hypertension and non H-type hypertension were 71.4±11.9 and 67.0±11.2 years. Creatinine (105.9±47.1)μmol/L vs. (88.6±55.9)μmoi/L (P〈0.001), uric acid (360.2±106.2)mmol/L vs. (309.4±83.8)mmol/L (P〈0.001), were higher in H-type hypertension, while eGFR (57.1±5.0)ml ·min^-1·(0.73 m2)^-1 vs. (68.8±23.4)ml·min^-1·(1.73 m2)^-1 (P〈0.001), was lower. The regression analysis found that Hey level was positively related with creatinine (β=1.542, P〈0.001) and uric acid (β=2.845, P〈0.001), while negatively related with eGFR (β=-0.726, P〈0.001 ) in primary hypertension patients. Furthermore, Hey level was also positively related with creatinine (β=1.028, P〈0.001) and uric acid (β=1.661, P〈0.001), while negatively related with eGFR (β=-0.482, P〈0.001) in H-type hypertension patients. Conclusion Hey level is associated with renal function in primary hypertension patients, especially in H-type hypertension patients.
