摘要 目的探讨3.0T动态增强磁共振扫描(DCE-MRI)联合弥散加权成像(DWI)诊断三阴性乳腺癌的临床价值。方法收集我院2011年9月-2013年9月收治的经病理证实为三阴性乳腺癌(观察组)18例、非三阴性乳腺癌患者(对照组)58例,回顾性分析其DCE-MRI及DWI图像资料。对比两组患者肿瘤病灶的强化模式、TIC-曲线特征及ADC值3项指标的差异。结果 (1)MRI强化特点:观察组以环形强化为主要特征,而对照组以不均匀强化为主要特点,差异具有统计学意义(P<0.05)。(2)TIC-曲线观察组多为流出型,对照组多为流出型及平台型,差异无统计学意义(P>0.05)。(3)DWI通过测量ADC值来量化分析,观察组ADC值较对照组低,差异有统计学意义(P<0.05)。结论三阴性乳腺癌以环形强化为主、ADC值较低,联合3.0T DCE-MRI及DWI有助于提高对三阴性乳腺癌的诊断。 Objective To discuss the clinical study of 3.0T dynamic contrast-enhanced magnetic resonance (DCE-MRI) in combination with diffusion-weighted imaging(DWI) in diagnoses of triple negative breast cancer. Methods Eighteen cases of triple negative(observation group,OG) and fifty-eight cases of non-triple negative breast cancer (control group,CG),confirmed by pathol-ogy,are enrolled in this study,from September 2011 to September 2013. We perform a retrospective analysis of these two groups patients’ imaging data of DCE-MRI and DWI. We compare the enhancement features,TIC-curve and ADC value between these two groups. Results (1)Ring enhancement as the main feature appears in OG,and irregular enhancement in CG. The difference is statistically significant(P〈0.05). (2) TIC-curves show outflows in OG and outflows platform type in CG,mostly. There is no signifi-cant differences between the two groups(P 〉0.05). (3) DWI quantitatively analysis by measuring the ADC value. And the ADC value of OG is smaller than that of CG. The difference is statistically significant (P〈0.05). Conclusion The three negative breast cancer&amp;nbsp;mostly shows ring enhancement with low ADC value. The combination of 3.0T DCE-MRI and DWI is helpful to improve the diag-noses of triple negative breast cancer.
