早期呼吸支持救治急性中毒所致呼吸衰竭的临床研究 被引量:7

The clinical experience of early respiratory support in therapy of respiratory failure caused by acute poisoning

摘要 目的探讨早期呼吸支持救治急性中毒所致呼吸衰竭的临床疗效及安全性。方法回顾性收集本院急诊内科2012年1月至2014年2月收治的95例急性中毒所致呼吸衰竭患者的临床资料,试验组50例患者在存在自主呼吸条件下,依据昏迷、发绀等临床症状以及呼吸监测指标,早期给予气管插管机械通气;对照组45例患者在呼吸停止后5分钟内行气管插管机械通气;分析两组患者的临床痊愈率、呼吸机使用时间及相关并发症发生情况。结果试验组患者临床痊愈率[92.0%(46/50)]显著高于对照组[75.6%(34/45)](P=0.028),试验组患者呼吸机使用时间[(47.3±8.2)小时]明显少于对照组[(86.5±10.8)小时](P<0.001);试验组患者肺部感染和低血压发生率[58.0%(29/50),4.0%(2/50)]均显著低于对照组[82.2%(37/45),17.8%(8/45)](P=0.010,P=0.029)。结论早期呼吸支持救治急性中毒所致呼吸衰竭临床效果确切,不仅可有效提高抢救成功率,而且能够降低机械通气相关并发症的发生率。 Objective To investigate the clinical therapeutic effect and security of early respiratory support in therapy of respiratory failure caused by acute poisoning. Method The clinical data of 95 patients with respiratory failure caused by acute poisoning, hospitalized in our hospital from January 2012 to February 2014 were collected retrospectively. In this study, there were 50 patients in experimental group received early mechanical ventilation according to clinical symptoms and respiratory monitoring indicators in the presence of the spontaneous breathing, and 45 patients in control group received endotracheal intubation within 5 minutes after the cessation of breathing. The clinical cure rate, ventilation time and the incidence of complications were analyzed and compared between two groups. Result The clinical cure rate of patients in experimental group [92.0%(46/50)] was significantly higher than that in control group [75.6%(34/45)](P = 0.028), and the ventilator using time of patients in experimental group [(47.3±8.2) hours] was also significant lower than that in control group [(86.5±10.8) hours](P〈0.001); both the incidence of pulmonary infection and low blood pressure of patients in experimental group [58.0%(29/50), 4.0%(2/50)] were significant lower than that in control group[82.2%(37/45), 17.8%(8/45)](P = 0.010, P = 0.029). Conclusion Early respiratory support has excellent clinical therapeutic effect on respiratory failure caused by acute poisoning, which can not only increase the cure rate of rescue, but also reduce the incidence of ventilator-associated complications.
