摘要
目的探讨结石成分和CT值对经皮肾镜超声碎石清石效率的影响。方法分析经皮肾镜超声碎石术治疗的56例上尿路结石患者的结石CT值、结石成分与超声碎石清石术的临床资料。结果草酸钙和磷酸钙等CT值高的结石(CT值≤1 500 HU),瑞士EMS第5代超声碎石清石系统仍可碎石清石,但碎石速度缓慢。CT值越低碎石速度越快,相反则慢,但对于少数含有草酸尿酸盐混合结石,由于结石表面结构光滑,即使CT值≤963.42 HU时,由于超声探针很难固定结石而不易被击碎,需先用气压弹道探针将结石外壳破碎再行超声碎石清石,碎石速度明显缓慢。结论经皮肾镜超声碎石清石速度与结石CT值和结石成分关系密切。术前测定结石的CT值,可作为预测和评估超声碎石效率的参考指标之一。
[ Objective ] To investigate the effect of stone composition and CT value on percutaneous nephro- scope ultrasonic lithotripsy efficiency. [Methods] The clinical data of the 56 cases of upper urinary stones, stone composition and CT value of ultrasonic lithotfipsy under percutaneous nephroscope ultrasonic lithotripsy were ana- lyzed. [ Results ] Calcium oxalate and calcium phosphate stone with high CT value (CT ≤ 1 500 HU), Switzerland EMS fifth generation ultrasound lithotripsy system still cleared rubble stone, but slowly. The lower the CT gravel faster, but slow for a few contain oxalic acid uric acid salt stone, because stone surface smooth, even if CT≤963.42 HU, because the ultrasonic probe was difficult to be fixed and not easy to crush stones, the stones shell crushing and ultrasonic lithotripsy with pressure ballistic probe, gravel apparent showed slow pace. [ Conclusion ] Percutaneous nephroscope ultrasonic lithotripsy was closely related with stone speed CT value and stone composition. Preoperative determination of stone CT value can be used as one of the reference index to predict and evaluate the efficiency of ultrasonic lithotripsy.
