摘要
目的观察桥本氏甲状腺炎(HT)患者血清中1,25(OH)2D3的水平与甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)的关系,并探讨补充骨化三醇对甲减患者血清FT3、FT4、TSH、TPOAb、TGAb水平的影响。方法选取未经治疗的HT患者46例及同期健康体检者40例作为观察组和对照组,检测其血清FT3、FT4、TSH、TPOAb、TGAb水平。另外选择HT组血清1,25(OH)2D3低于正常值的患者,分为两组,HTl组仅口服左旋甲状腺素;HT2组除口服左旋甲状腺素外,加用骨化三醇口服,连续2个月,检测患者血清TGAb、TPOAb、FT3、FT4、TSH、1,25(OH)2D3水平。结果⑴HT甲减组血清1,25(OH)2D3明显低于正常组(P<0.05)。⑵两组血清1,25(OH)2D3治疗后均较治疗前升高(P<0.05);治疗后血清1,25(OH)2D3水平HT2组较HT1组明显升高(P<0.05);两组TGAb及TPOAb治疗后均较治疗前降低,且HT2组下降幅度优于HT1组(P<0.05);治疗后HT2组较HT1组甲状腺功能明显改善(P<0.05)。结论 HT甲减患者存在1,25(OH)2D3缺乏,提示1,25(OH)2D3在甲状腺自身免疫过程中发挥一定作用;补充维生素D具有降低甲状腺自身抗体,抑制甲状腺自身免疫反应的作用。
Objective To investigate the relationship between the levels of serum of 1 ,25(OH)2 D3 and thyroid peroxidase antibody (TPOAb) ,thyroglobulin antibody (TGAb) in Hashimoto’s thyroiditis (HT ) patients ,and explore complementary ossification calcitriol assist in the treatment of Hashimoto’s hypothy‐roidism on serum TPOAb ,TGAb ,FT3 ,FT4 ,TSH .Methods choose the HT patients whom without treat‐ment as 46 cases ,and 40 healthy cases of the same period as controls to detect serum TPOAb ,TGAb ,FT3 , FT4 ,TSH levels .And also choose the HT patients whose serum 1 ,25(OH)2 D3 was below the normal ,fell into two groups ,HTl group based on application of levothyroxine alone .HT2 group ,nut only based on the application of levothyroxine ,but also based on ossification for oral estriol ,continue for two months to detect the level of serum TGAb ,TPOAb ,FT3 ,FT4 ,TSH ,1 ,25(OH)2 D3 .Results The serum1 ,25(OH)2 D3 of HT hypothyroidism was significantly lower than the normal group (P〈 0 .05) .After treatment ,the level of serum 1 ,25(OH)2 D3 has increased in both of the two group (P 〈 0 .05 ) .After treatment ,serum 1 ,25 (OH)2 D3 level in HT2 group was significantly higher than the HT1 group (P〈 0 .05) .After treatment ,the level of TGAb and TPOAb in the two group has been declined ,and HT2 group declined obviously than HT1 group (P〈 0 .05) .Compared to HT1 group ,the thyroid function of HT2 group after treatment improved significantly (P〈 0 .05) .Conclusion HT patients with hypothyroidism exists 1 ,25(OH)2 D3 deficiency ,sug‐gesting that it plays a role in thyroid autoimmunity in the process .The supplement of vitamin D may help to reduce thyroid autoantibodies and inhibit the thyroid autoimmune response .
