摘要
目的：评估腹腔镜子宫肌瘤切除手术难度评分系统在腹腔镜子宫肌瘤切除手术中应用效果。方法：采用随机数字表法选取我院100例采用腹腔镜切除子宫肌瘤的患者,采用手术难度评分系统评估,根据不同得分按照等级分组,观察并比较各组患者手术的临床效果。结果：采用腹腔镜子宫肌瘤切除手术难度评分系统进行评分〈5分组,5-7分组,≥8分组进行腹腔镜子宫肌瘤切除手术所用时间,术前术后血红蛋白变化值,术后肛门排气所用时间有明显差异,评分〈5分组手术时间≤100min者占（68.89%）,5-7分组手术时间≤100min者占（35.00%）,≥8分组手术时间≤100min者占（35.00%）,具有明显差异,统计学上有意义（P〈0.05）。结论：腹腔镜子宫肌瘤切除手术难度评分系统在腹腔镜子宫肌瘤切除手术中具有指导价值,评分越低手术难度越低,了解手术难易程度有助于手术成功进行,应该在临床上广泛应用。
Objectives： To evaluate the application of laparoscopic myomectomy operation difficulty scoring system in laparoscopic myomectomy. Methods： 100 patients received laparoscopic resection of uterine leiomyoma in our hospital were randomly selected and evaluated with the difficulty scoring system. Groups were divided according to the different score,and the clinical effects of patients in each group were observed and compared. Results： There was significant difference in the operation time,value of hemoglobin changes before and after surgery,postoperative exhaust time between groups whose scores of the laparoscopic myomectomy operation difficulty scoring system were〈5,5 - 7 and ≥ 8. For score 5 group,5 - 7 group and ≥ 8 group,the patients with operation time ≤ 100 minutes accounted for（ 68. 89%）,（ 35%） and（ 35%） respectively,with a statistically significant difference（ P〈 0. 05）.Conclusion： The laparoscopic myomectomy operation difficulty scoring system has reference value in laparoscopic myomectomy,lower scores coming with lower operation difficulty. To understand the operation difficulty contributes to the success of operation,which should be widely applied in clinical practice.
