局部枸橼酸抗凝在连续性肾脏替代治疗中的应用 被引量:40

摘要 体外循环抗凝技术是保证连续性肾脏替代治疗(continuous renal replacement therapy,CRRT)能否顺利、有效进行的关键的环节,但也是CRRT面临的最严峻的挑战之一。临床上,需要接受CRRT治疗的危重病患者往往存在活动性出血、近期重大手术、严重创伤、凝血功能障碍等高危出血状况,传统的全身肝素化抗凝可显著增加出血风险甚或造成肝素诱导血小板减少,而无肝素抗凝往往因为体外循环管路和滤器凝血导致治疗中断,并且治疗过程中需要定时使用生理盐水冲管,极大程度上影响到CRRT的治疗量和透析充分性。
