还原型谷胱甘肽对重症急性胰腺炎肝损害的保护作用和机制探讨 被引量:12

摘要 目的探讨还原型谷胱甘肽(GSH)对重症急性胰腺炎(SAP)肝损害的保护作用和机制。方法将65例SAP合并肝功能损害者随机分为观察组33例和对照组32例,2组入院后均给予常规综合疗法,观察组在此基础上给予还原型谷胱甘肽注射液静滴,1次/d,连续7 d。比较2组治疗前后血尿淀粉酶、脂肪酶、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBil)、白蛋白(ALB)、γ-谷氨酰转肽酶(γ-GT)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-10、丙二醛(MDA)、过氧化物歧化酶(SOD)和谷胱甘肽过氧化物酶(GSH-Px)水平。结果治疗后2组血尿淀粉酶、血清脂肪酶、ALT、AST、TBil、ALB、γ-GT、TNF-α、IL-6、IL-10、MDA、SOD和GSH-Px均明显改善(P均<0.01),且观察组的改善幅度均显著大于对照组(P均<0.01)。观察组未发现与GSH注射液相关的不良反应。结论对于SAP合并肝损害患者,早期补充外源性GSH可通过抑制炎症反应、清除氧自由基、减轻脂质过氧化反应等机制来保护肝脏功能,促进病情恢复。 Objective It is to explore the protective effect and mechanism of reduced glutathioue (GSH) on severe acute panereatitis (SAP) with liver function lesion. Methods Sixty-five cases of patients with SAP and liver function lesion were randomly divided into observation group (33 eases) and control group (32 cases). The two groups both received routine com- prehensive therapy when admitted to hospital. On this basis, the observation group received GSH injection by intravenous drip infusion once per day and for 7 d. The levels of serum and urine amylase, lipase, glutamic-pyruvie transaminase (ALT) , glu- tamic oxalacetie transaminase ( AST), total bilirubin ( TBil), albumin ( ALB), T - glutamyltranspeptidase ( T - GT), tumor necrosis factor - ot ( TNF -α) , interleukin - 6 ( IL - 6), IL - 10, malonaldehyde ( MDA), superoxide dismutase ( SOD ) and glutathion peroxidase ( GSH - Px) of two groups were compared before and after treatment. Results Serum and urine amylase, lipase, liver function, TNF -α, IL - 6, IL - 10, MDA, SOD and GSH - Px were all obviously improved after treatment in both groups (P 〈 0.01 ) , and the improvements in observation group were more obvious than that in control group (P 〈 0.01 ). No obvious adverse reaction related with GSH injection in observation group was found. Conclusion For SAP with liv- er function lesion, the supplement of exogenous GSH at an early stage can protect liver function and be useful in improvement for prognosis. The effect is realized by suppressing inflammation, scavenging oxygen free radicals and relieving lipid peroxida- tion reaction.
