摘要
目的:研究左乙拉西坦对难治性癫痫患者外周血主穹窿蛋白(MVP)表达的影响,探讨其在难治性癫痫中的作用。方法:收集2012年09月至2013年02月无锡市儿童医院难治性癫痫患者30例,男18例,女12例,年龄(5.2±1.8)岁。这些患者均口服左乙拉西坦超过12个月,分为用药前(B组)、用药6个月(C组)和12个月(D组)3组。选取同期来院体检的健康儿童30例为对照组(A组),男16例,女14例,年龄(5.5±2.9)岁。应用半定量逆转录聚合酶链反应(RT-PCR)方法检测外周血中MVP基因,免疫蛋白印记(Western Blot)方法检测MVP蛋白的表达,结果经统计学处理。结果:空白对照组外周血中有少量MVP表达(mRNA:1.133±0.152、蛋白:0.020±0.008);难治性癫痫患者用药前MVP大量表达(mRNA:1.788±0.097、蛋白:0.209±0.012),与空白对照组比较有显著性差异(P<0.05)。左乙拉西坦治疗组在用药后6个月(mRNA:1.657±0.126,蛋白:0.196±0.012)、12个月(mRNA:1.466±0.199,蛋白:0.180±0.008)均较对照组减低(P<0.05);用药6个月、12个月两组比较有显著性差异(P<0.05)。结论:MVP可能参与癫痫耐药的发生,LEV可以控制癫痫发作,并可能下调MVP的表达。
Objective : To investigate the effects of levetiracetam on expression of major vault protein (MVP) in the peripheral blood of children with refractory epilepsy. Methods: From Sep. 2012 to Feb. 2013, 30 patients with refractory epilepsy ( 18 male cases, 12 female cases, age 5.2 ± 1.8 years) were collected in Chirdren's Hospital of Wuxi City. The patients had to take levetiracetam over twelve months. They were divided into three groups : Before taking drug group ( B group), taking drug for six months group( C group) and taking drug for twelve months group( D group). The 30 healthy children (16 male cases, 14 female cases) to the hospital physical examination were collected as control group ( A group). The expression of MVP mRNA in their peripheral blood cells was detected by semi-quantitative reverse transcrip- tion- polymerase chain reaction (RT-PCR), and their protein was detected by Western Blot. The results were processed by statistics. Results: A small amount of MVP expression was detected in the control group (A group, mRNA: 1. 133 ±0. 152, Protein:0. 020 ± 0. 008 ). MVP protein expression elevated significantly before treatment (B group, mRNA: 1. 788 ± 0. 097, Protein: 0. 209± 0.012). And there was significant difference as for the MVP expression in between the A group and the B group according to statistics expression trends(P 〈 0.05). The MVP expression in levetiracetam treatment groups(C group, mRNA: 1. 657 ± 0. 126, Protein: 0. 196 ± 0. 012; D group, mRNA: 1. 466 ± 0. 199, Protein: 0. 180 ±0. 008) was lower than A group(P 〈0.05 ). There was significant difference as for the MVP expression in between the C group and the D group according to statistics (P 〈 0.05 ). Conclusion: MVP may play an important role in the development of resistance of refractory epilepsy, LEV can control epileptiform seizure, and reduce the MVP expression.
