摘要
目的 探讨氨基末端脑钠肽前体(NT-proBNP)与冠状动脉病变的相关性.方法 入选笔者医院2012年6月~2014年2月诊断为急性ST段抬高型急性心肌梗死(ST-elevation myocardial infarction,STEMI)患者157例,根据患者血中NT-proBNP水平分为NT-proBNP正常组(NT-proBNP <780pg/ml) 78例、NT-proBNP升高组(NT-proBNP> 780pg/ml) 79例.统计患者入院基本临床资料,所有患者均检测肌钙蛋白(cTnⅠ)、空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C),根据冠状动脉狭窄直径≥50%累及左前降支(LAD)、左回旋支(LCX)、右冠状动脉(RCA)或左主干(LM)分为单支、双支(累及左主干为双支病变)及3支病变组,根据Gensini积分标准对每位患者冠状动脉病变进行评分.计量资料采用均数±标准差((x±s)表示,组间比较采用两独立样本t检验,计数资料采用率表示,组间比较用x2检验,NT-proBNP水平与冠状动脉Gensini积分相关性采用线性相关分析.结果 与NT-proBNP正常组相比,NT-proBNP升高组患者冠状动脉病变情况在总体上差异有统计学意义(P =0.000),以冠状动脉无病变组作为对照组,单支病变组、两支/左主干病变组和3支病变组与对照组相比,差异均有统计学意义(P=0.004,P=0.000,P=0.000);NT-proBNP升高组患者平均冠状动脉病变支数更多(1.22±0.84 vs2.08±0.82,P=0.000),Gensini积分更高(24.83±22.31 vs 54.60±29.13,P=0.000);NT-proBNP与Gensini积分存在相关关系(r=0.648,P=0.000).结论 冠状动脉病变程度与NT-proBNP存在一定关系,可以用血中NT-proBNP水平预测冠状动脉病变程度.
Objective To explore the correlation between NT - proBNP and coronary arterty lesions. Methods Totally 157 patients were diagnosed coronary heart disease (CHD) from June 2012 to February 2014 in our hospital. Patients were divided into NT - proBNP normal group (78 cases) or NT- proBNP elevatory group(79 cases) according to the level of NT -proBNP. 157 patients diagnosed of CHD were studied basic clinical charateristics, cTnI, fasting blood glucose,total cholesterol, triglycerides, high density lipoprotein choles- terol and low density liporotein cholesterol were determined in all the patients. All patients were divided into none vessel lesions, 1 vessel lesions,2 vessels lesions or large main vessel lesions,3 vessels lesions groups according to stenosis of major vessels≥50%. We evaluated every patient coronary lesions according to Gensini score criteria. The quantitative data were expressed by x ± s and the qualitative data were expressed by rete. The quantitative data comparison between the groups was used independent - samples t test and the quantitative da- ta comparison between grounps was used ehi - square test, and the correlation between NT - proBNP and coronary artery lesions Gensini score was used linear correlation analysis. Results On the whole, there is statistical significance (P = 0. 000) in Coronary artery lesions in patients with NT -proBNP normal group and NT -proBNP group. Compared with none coronary atery lesions, 1 vessel lesions,2 or left main vessels lesions and 3 vessel lesions have statistical significance ( P = 0. 004, P = 0. 000, P = 0. 000 ). Compared with NT - proBNP normal group,the average coronary artery lesions and Gensini score of every patients in SAP group is higher( P 〈 0.05 ). NT - proBNP was correlated with the Gensini score. Conclusion There is correlation between NT - proBNP levels and coronary artery lesions, and the NT - proBNP levels can be used to predict the severity of coronary artery lesions.
