摘要
目的探讨内脏脂肪组织丝氨酸蛋白酶抑制因子(Vaspin)水平与冠心病(CVD)的相关性。方法随机选取在上海市第十人民医院心内科就诊并行冠状动脉造影的191例被试,其中有88例确诊CAD的患者,103例为阴性。检测常规生化、心肌标志物、Vaspin和脂连素(adiponectin),通过超声心动图了解心脏功能,并对该组人群进行2年的随访,研究其与基线临床参数和Vaspin水平之间的关系。结果 CAD组Vaspin水平显著低于非CAD组[0.18(0.09,0.46)ng/ml vs 0.43(0.20,1.81)ng/ml,P<0.001];经过2年的随访,共有41人49次心脏不良事件(MACE)发生,MACE组基线Vaspin显著低于无MACE组[0.23(0.07,0.69)ng/ml vs 0.22(0.11,0.46)ng/ml,P=0.031];在所有MACE人群中Vaspin平均水平随着MACE的严重性加剧而降低,最低为死亡患者[(0.18±0.08)ng/ml];最后,左室收缩功能下降的患者基线Vaspin水平显著低于收缩功能上升者[0.18(0.13,0.59)ng/ml vs 0.53(0.41,0.25)ng/ml,P=0.001]。结论 CAD患者血清Vaspin水平较低,2年内发生MACE的患者基线Vaspin水平较低,心功能下降者基线Vaspin水平较低。
Objective To explore the association between the visceral adipose tissue-derived serine protease inhibitor (Vaspin) level and coronary atherosclerotic heart disease (CAD). Method A total of 191 patients angiographically proved were randomly enrolled in this study, including 88 patients with CAD and 103 subjects without CAD. Circulating Vaspin, adiponectin, clinical parameters were assayed. The heart function was evaluated according to the eehocardiography. Enrolled subjects were followed for 2 years. The relationship between serum Vaspin concentration of baseline and the outcomes of CAD was evaluated. Result The Vaspin concentration in CAD patients was significantly lower than that in non-CAD patients [ 0. 18 (0.09,0.46) ng/ml vs 0.43 (0.20,1.81) ng/ml, P〈0. 001 ]. After 2-years' follow-up, there were 49 times major adverse cardiac events occurred with 41 patients. Vaspin level in MACE patients was dramatically lower than that in non-MACE patients [ 0.23 (0.07,0.69) ng/ml vs 0. 22 (0.11,0.46) ng/ml, P= 0.031 ]. The patients who had the lowest concentration of baseline Vaspin were attacked by cardiac death[ (0.18±0.08) ng/ml]. Baseline Vaspin level in the patients whose left ventricular ejection fraction decreasedwas significantly lower than that in the patients whose LVEF rose up[0.18 (0.13,0.59) ng/ml vs 0.53 (0.41,1.25) ng/ml, P=0. 001 ]. Conclusion Circulating Vaspin is lower in CAD patients. Patients who occurred MACE and had decreased LVEF have lower baseline Vaspin level during 2- years' follow-up.
