摘要
目的探讨中药黄芪注射液穴位注射对痉挛型脑性瘫痪患儿肌肉电生理及影像学的影响。方法符合纳入标准的痉挛型脑瘫患儿,随机分为黄芪注射液穴位注射组(治疗组)及非穴位注射组(对照组),治疗组89例,对照组90例。治疗组采用综合康复治疗加黄芪注射液穴位注射,对照组采用综合康复治疗,疗程均为3个月,对治疗前后进行腓肠肌肌肉超声、表面肌电图及GMFM量表评分比较。结果经3个月治疗后:1在表面肌电图比较中,治疗组腓肠肌波幅均值有提高,腓肠肌肌电积分值有降低,治疗前后比较有统计学意义(P<0.05),治疗组优于对照组(P<0.05);2在超声比较中,治疗组中腓肠肌肌肉纤维结构紊乱好转4例;对照组中肌肉纤维结构紊乱好转3例;治疗组中肌肉回声增强好转3例;对照组中肌肉回声增强好转2例,治疗前后比较无统计学意义(P>0.05),与对照组比较差异无统计学意义(P>0.05);3在GMFM评分比较中,治疗组与对照组治疗前后评分均有明显提高,有统计学意义(P<0.05),治疗组优于对照组(P<0.05)。结论中药黄芪注射液穴位注射能够使痉挛型脑瘫患儿肌肉收缩时参与的肌纤维数量增加,超声影像显示肌肉纤维走行及回声有好转趋势。提示该疗法能够改善肌肉萎缩程度,提高肌肉电活动,增加肌肉收缩、舒张活性,进而使脑瘫患儿运动能力明显提高。
Objective To investigate the effect of Traditional Chinese medicine Astragalus Injection on the children with cerebral palsy muscle electrophysiology and imaging. Methods 2012. 1 ∽ 2013. 5 were in accordance with our inclusion criteria of children with spastic cerebral palsy,according to the number of hospitalized,randomly divided into acupoint injection with astragalus injection group( treatment group) and non acupoint injection group( control group),89 cases in the treatment group,90 cases in the control group. The treatment group received rehabilitation training,acupuncture,massage,physical therapy and acupoint injection with astragalus injection,the control group received rehabilitation training,acupuncture,massage,physical therapy,treatment for 3 months,before and after the treatment were compared with gastrocnemius muscle ultrasound,surface EMG and GMFM score. Results 1after 3 months after treatment: in the surface EMG in treatment group comparison,gastrocnemius muscle the mean amplitude was increased,the gastrocnemius muscle EMG decreased,there was significant difference before and after treatment(P〈0. 05),there was statistically significant difference compared with the control group(P〈0. 05),before and after the treatment of control group also has obvious change before and after treatment,with significant differences(P〈0. 05); 2 in ultrasound,4 cases improved gastrocnemius muscle fiber structure disorder in the treatment group in the control group; disorders of muscle fiber structure improvement in 3 cases in treatment group; muscle echo enhancement improved in 3 cases; in the control group,muscle echo enhancement improved in 2 cases,treatment compared with no statistical significance(P〈0. 05),and the control group had no significant difference( P O. 05); 3the GMFM score comparison,the treatment group and the control group before and after treatment scores were significantly increased,with statistical significance(P〈0. 05),the treatment group had statistical significance compared with the control group(P〈0. 05). Conclusion The number of muscle fibres of Traditional Chinese medicine Astragalus injection can make children with spastic cerebral palsy muscle involvement increases,the ultrasound image of muscle fiber and echo has improved trend. The results suggest that the therapy can improve muscle atrophy degree,improve the electrical activity of muscle,increase muscle contraction,relaxation activity,thus significantly improve the ability in children with cerebral palsy.
