428例开放性骨折伤口感染的细菌培养及药敏试验结果分析 被引量:22

摘要 目的探讨开放性骨折伤口感染的病原菌分布及耐药情况,为临床合理使用抗菌药物提供科学依据。方法对医院2009年1月-2014年4月开放性骨折患者伤口分泌物分离的462株病原菌的细菌培养和药敏试验结果进行统计分析。结果 428例标本中356例有细菌生长,占83.18%,共分离出病原菌462株。其中122例标本同时分离出2种或2种以上病原菌,占阳性标本的34.27%;革兰阴性菌281株,占60.82%,其中最常见的致病菌是大肠埃希菌和铜绿假单胞菌;革兰阳性菌166株,占35.93%,其中最常见的是金黄色葡萄球菌和凝固酶阴性葡萄球菌;真菌15例,占3.25%。药敏试验结果显示分离的所有病原菌对万古霉素的耐药率最低,均<2.00%;对亚胺培南的耐药率均<15.00%;对青霉素的耐药率均>75.00%。结论临床医师应根据患者标本的细菌分布特征和药敏试验结果,制定科学合理的用药方案,采取有效措施积极预防和控制感染。 Objective To investigate the bacterial culture and drug resistance in open fracture infection,to provide the scientific evidence for clinical rational use of antibiotics. Methods 462 strains of pathogenic bacteria isolated from hospitalized patients with open fracture infection from Jan. 2009 to Apr. 2014 were analyzed statistically for the results of bacteria culture and susceptibility testing. Results 356 cases of bacterial growth in 428 cases(83. 18%),462 pathogenic bacterial strains were isolated,and so did 122 cases growth of two or more than two kinds of pathogens,accounted for 34. 27% of the negative specimens,the pathogenic bacterial including 281 Gram- negative bacilli(60. 83%),the most common pathogenic bacteria were E.coli and P. aeraginosa; There were 166 Gram- positive bacilli(35. 93%),the most common pathogenic bacteria were Staphylococcus and coagulase negative Staphylococcus aureus; 15 fungi accounted for 3. 25%. The drug susceptibility testing demonstrated that the drug resistance of all pathogenic bacteria were less than 2. 00% to vancomycin,were less than 15. 00% to imipenem,the resistance rate to penicillin were all higher than 75. 00%. Conclusion The clinical doctors should prescribe the antibiotics reasonably according to etiology and drug susceptibility,take effective measures to prevent and control infection.
