摘要
目的:分析脉冲振荡法(IOS)与常规肺功能(PFT)各参数的相关性,研究IOS测定呼吸阻抗的重复性,探讨IOS在慢性阻塞性肺疾病(COPD)中的应用价值。方法:根据入选标准和排除标准共入组30例稳定期COPD患者及11例对照组,用常规肺功能及IOS法进行测定。将IOS与常规肺功能各参数进行比较,使用ROC曲线判断对COPD诊断有意义的参数;同时重复测定稳定期COPD患者IOS各值。结果:1COPD组与对照组IOS各参数相比R5、R5-20、X5、Fres及Alx,差异均有统计学意义,R20差异无统计学意义。2COPD患者R5-20与FEV1、PEF呈显著负相关,Fres与FVC、FEV1、PEF、MMEF%呈显著负相关,而与RV/TLC呈显著正相关,X5与RV、RV%及RV/TLC呈显著负相关,Alx与RV%及RV/TLC呈显著正相关。3COPD组患者重复测定IOS各参数比较均差异无统计学意义。4曲线下面积:R5-20>Fres>Alx>X5>R5。结论:IOS与PFT各参数有很好的相关性,不仅能敏感的反映COPD的气道变化,同时可反映COPD患者的肺容量变化,且具有很好的重复性。X5、R5-20、Fres、Alx、R5对COPD的诊断有一定的准确性,其中R5-20是诊断准确性最佳的参数。
Objective:Analyzing the correlation between each IOS parameters and PFT parameters; Researching the repeatability of IOS is used to measure respiratory resistance ; Probing the application value of ISO in chronic pulmonary obstructive disease(COPD). Methods: According to the inclusion criteria and exclusion criteria, 30 stable phase COPD patients and 11 control groups are selected, tested by pulmonary function test and IOS test. Estimating COPD meaningful parameters by using ROC curves; Duplicate test IOS values of sta- ble phase COPD. Results : 1. A statistical difference between IOS parameters of COPD group and IOS parame- ters of control group on R5 ,RS-20,XS,Fres,Alx, no statistical difference on R20. 2. For COPD patients, a significant negative correlation between R5-20 and FEV1 ,PEF , Fres and FVC ,FEV1 ,PEF,MMEF%, )(5 and RV ,RV% ,RV/TLC; a significant positive correlation between Alx and RV% ,RV/TLC. 4. Area under curve: R5-20 〉 Fres 〉 Ah 〉 X5 〉 R5. Conclusion : A good correlation between each IOS and PFT parameters, not only this could sensitively reflect the changes of COPD airways, but also could reflect the changes of lung volume of COPD patients and possess a good repeatability. X5 ,RS-20,Fres,Alx,R5 have certain help on the accuracy of COPD's diagnosis, R5-20 are the best parameter for diagnosis'accuracy measuring.
