摘要
目的探讨吻合第一掌背动脉Kite皮瓣桡侧神经增强拇指指腹感觉功能的临床应用价值。方法以2008年1月~2013年12月采用吻合第一掌背动脉Kite皮瓣桡侧神经的方法治疗拇指指腹皮肤缺损患者40例为实验组,采用吻合第一掌背动脉Kite皮瓣桡侧神经的方法增强拇指指腹感觉功能;以2005年1月~2007年12月收治的拇指指腹皮肤缺损患者30例作为对照组,不吻合kite皮瓣桡神经。所有患者均记录指腹感觉恢复功能和术后皮瓣周围外观及有无神经瘤形成。结果 1实验组seddon感觉评定减少率显著高于对照组,组间差异有统计学意义(P〈0.05)。2两组患者拇指皮瓣外观良好,功能满意,色泽与周围肤色相近。3两组患者术后神经瘤形成率差异无统计学意义(P〉0.05)。结论吻合第一掌背动脉Kite皮瓣桡侧神经可以更大限度地增强拇指指腹感觉,降低患者伤残率,更好地改善患者的身心健康。
Objective To analyze radial nerve of the first dorsal metacarpal artery flap Kite anastomosis in enhancing the thumb on application of sensory function.Methods 40 patients with skin defects thumb treated with anastomosis first dorsal metacarpal artery flap radial nerve Kite method were selected as the experimental group.30 patients with skin defects thumb were selected as a control group without kite flap radial nerve.Results The Seddon feeling reduction rate of the experimental group was significantly higher than that of the control group(P〈0.05).The thumb flap appearance and function were satisfactory.The postoperative neuroma formation rate was no significant difference(P〉0.05).Conclusion The radial nerve of Kite flap's first dorsal metacarpal artery anastomosis can enhance a greater extent thumb feeling,reduce patient disability rates and improve the patient's physical and mental health.
