综合干预措施对降低老年骨科手术患者医院感染效果分析 被引量:7

Effect of comprehensive interventions on reduction of incidence of nosocomial infections in elderly patients of orthopedics department

摘要 目的探讨综合干预措施对降低老年骨科手术患者医院感染和改善术后恢复的临床效果,为寻找有效医院感染预防控制措施奠定理论基础。方法对2011年2月-2013年6月收治的1 268例患者按照随机数字表法分为观察组和对照组各634例,对照组给予常规干预,观察组在对照组干预的基础上给予综合干预措施,比较两组患者的医院感染发生率、术后疼痛程度、术后免疫功能。结果患者术后医院感染率观察组为0.63%,对照组为1.89%,差异有统计学意义(P<0.05);观察组术后第2天SAS及SDS评分均低于对照组,差异有统计学意义(P<0.05);观察组住院、下床活动、伤口拆线时间、住院费用均低于对照组,差异有统计学意义(P<0.05);观察组CD3+、CD4+及CD4+/CD8+比值均显著升高,CD8+显著降低,与对照组比较差异均有统计学意义(P<0.05)。结论综合干预措施降低了老年骨科手术患者医院感染率,缩短了术后恢复时间,降低了住院费用,减轻了术后疼痛程度,效果显著。 OBJECTIVE To explore the clinical effect of comprehensive interventions on reduction of incidence of nosocomial infections in elderly patients of orthopedics department and on the improvement of postoperative recov‐ery so as to lay theoretical foundation for effective prevention and control of the nosocomial infections .METHODS From Feb 2011 to Jun 2013 ,a total of 1 268 patients were enrolled in the study and randomly divided into the ob‐servation group and the control group ,with 634 cases in each group ;the control group was treated with the con‐ventional interventions ,and the observation group was given the comprehensive interventions based on the conven‐tional interventions ;the incidence of nosocomial infections ,postoperative pain degree ,and postoperative immune function were observed and compared between the two groups .RESULTS The incidence of postoperative nosocomi‐al infections was 0 .63% in the observation group ,1 .89% in the control group ,there was statistically significant difference (P〈0 .05) .Two days after the surgery ,both the SAS score and SDS score of the observation group were lower than those of the control group ,there was significant difference (P〈 0 .05) .The length of hospital stay ,ambulation time ,and wound suture removal time of the observation group were significantly shorter than those of the control group ,and the hospitalization cost of the observation group was significantly less than that of the control group (P〈0 .05) .The levels of CD3 + and CD4 + and the ratio of CD4 + /CD8 + of the observation group were significantly elevated ,while the level of CD8 + was remarkably reduced ,as compared with the control group (P〈0 .05) .CONCLUSION The comprehensive interventions can reduce the incidence of nosocomial infections in the elderly patients of orthopedics department ,shorten the postoperative recovery time ,reduce the hospitalization cost ,and relieve the pain ,with the effect remarkable .
