摘要
目的:探讨和比较三维彩声与核磁共振在胎儿畸形诊断中的应用价值,为临床早期诊断、制定优生优育方案提供可靠的理论依据。方法:回顾分析2012年3月-2014年3月来本院进行产前B超检查异常的孕妇120例,分别使用三维彩声和核磁共振对胎儿畸形进行诊断,随访至产后或者引产证实病例,分析对比两种诊断方法畸形胎儿的检出率、敏感性、特异性以及阳性预测值,以了解三维彩声与核磁共振在胎儿畸形诊断中的不同。结果:在胎儿畸形诊断的过程中,三维彩声的检出率为67.92%(72/106),特异性为81.30%,敏感性为100%,阳性预测值为79.60%,阴性预测值为100%;核磁共振的检出率为84.91%(90/106),特异性为89.60%,敏感性为100%,阳性预测值为88.20%,阴性预测值为100%。核磁共振组在检出率、特异性以及阳性预测值等方面明显优于三维彩声组,两组相比差异具有统计学意义;其敏感性和阴性预测值均为100%,两组相比差异无统计学意义。结论:核磁共振在胎儿畸形的诊断中具有明显的优势,可联合三维超声为产前胎儿畸形进行诊断。
Objective: To investigate and compare the application value of the three-dimensional color Doppler and the nuclear magnetic resonance in the diagnosis of fetal malformation. Then, to provide some reliable theoretical basis for the early clinical diagnosis and the proposals of prenatal and postnatal care.Method: 120 pregnant women who came to our hospital for prenatal ultrasound and were diagnosed with abnormal results from March 2012 to March 2014 and investigated. Three-dimensional color Doppler and nuclear magnetic resonance were separately used in order to diagnose fetal malformation. The pregnant women were given follow-up to postpartum or confirmed cases of odinopoeia. To learn the differences between the three-dimensional color Doppler and nuclear magnetic resonance in the diagnosis of fetal malformation, the detection rate, sensitivity, specificity and positive predictive value of the two diagnostic methods were compared and analyzed.Result: During the diagnosis of fetal malformation, the detection rate of the three-dimensional color Doppler was 67.92% (72/106), the specificity was 81.30%, the sensitivity was 100%, the positive predictive value was 79.60% and the negative predictive value was 100%. The detection rate of the nuclear magnetic resonance was 84.91% (90/106), the specificity was 89.60%, the sensitivity was 100%, the positive predictive value was 88.20% and the negative predictive value was 100%.Conclusion: In the diagnosis of the fetal malformation, the nuclear magnetic resonance has more apparent advantages. It can be united with the three-dimensional color Doppler to conduct antenatal fetal malformations.
