摘要 以2型糖尿病为例,探讨慢性非传染性疾病体检策略优化的问题。以最大化体检效用为目标,建立马尔科夫决策模型。模型从全社会角度定义成本的评价函数,除考虑直接医疗成本外,还考虑了间接的非医疗成本。为了使模型具有更大的现实意义,模型还考虑了患者由于及时体检知晓病情后生活习惯改善这一因素。采用逆向归纳法求解,结果证明模型能有效优化体检周期。通过对比,证明改进的成本评价函数和生活习惯改善这两个因素确实对优化结果有显著影响。 Based on the case of Type 2diabetes,test optimization for noninfectious chronic disease was studied in this paper.Differing from previous disease model,markov decision process was built with the objective to maximize the test utility.Both direct and indirect medical cost were considered to define cost function from the entire society perspective.Living habits improvement after receiving test result was taken into the model to make the model have more realistic significance than before.Backward induction was conducted to solve this model.It was indicated that the model can solve the test optimization problem,cost function and living habits improvement have influence on the optimization results of test period,patients living quality and total cost.
