二维和四维超声在胎儿先天性心脏病超声诊断中的意义 被引量:2

The significance of two-dimensional and fourdimensional ultrasound screening for fetal congenital heart disease

摘要 目的:探讨二维和四维超声在胎儿先天性心脏病超声诊断中的意义.方法:运用二维和四维超声诊断技术对751例高危妊娠妇女进行产前常规的胎儿心脏检查,观察胎儿右室流出道切面、左室流出道切面、四腔心切面、三血管-气管切面、动脉导管弓及主动脉弓切面的声像图,并与引产解剖或出生后检查比对.结果:在所有孕妇中,二维和四维超声检查,分别检出79例和82例先天性心脏病胎儿,检出率为10.52%和10.92%.引产解剖和出生后检查结果为83例,四维超声误诊1例,漏诊2例,准确率97.59%;二维超声误诊6例,漏诊10例,准确率87.95%,两组准确率的差异具有统计学意义(P<0.05).结论:二维和四维超声均能较为准确地诊断妊娠期妇女胎儿的先天性心脏病,但四维超声的诊断效果优于二维超声,更加值得推广使用. AIM: To investigate the significance of two-dimen- sional and four-dimensional ultrasound screening for fetal congeni- tal heart disease (FCHD). METHODS: We used two-dimen- sional and four-dimensional ultrasound diagnostic techniques to do prenatal fetal heart routine examination for 751 cases of high-risk pregnant women, and observed the ultrasonogram of fetal right ventricular outflow section, left ventricular outflow section, four- chamber section, three vessels and trachea section, aortic arch and arterial arch section, and then they were compared with the inspection results of odinopoeia or birth. RESULTS: In all preg- nant women, it diagnosed 79 cases and 82 cases of FCHD by two- dimensional and four-dimensional ultrasound respectively, and the positive rates were 10.52% and 10.92% respectively. The in- spection result of odinopoeia or birth was 83 cases with FCHD, so four-dimensional ultrasound with 1 case misdiagnosed and 2 cases missed diagnosis, the accuracy was 97.59% ; two-dimensional ul- trasound with 6 cases misdiagnosed and 10 cases missed diag- nosed, accuracy was 87.95%. The differences of the accuracy between the two groups had statistical significance ( P 〈 0.05 ). CONCLUSION: Two-dimensional and four-dimensional ultra- sound both can diagnose fetal congenital heart disease of pregnant women accurately, but the diagnosis effect of four-dimensional ul- trasound is better than two-dimensional ultrasound, and it is wor- thier of promotion and application.
