急性呼吸窘迫综合征患者血管外肺水与肺顺应性相关性研究 被引量:13

摘要 急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)是严重感染、休克、创伤等疾病引起的肺毛细血管内皮和肺泡上皮弥漫性损伤,以进行性低氧血症、呼吸窘迫为特征的临床综合征,是重症监护病房(ICU)常见危重症.肺顺应性降低是ARDS的重要病理生理学特征,研究认为,ARDS患者急性期肺顺应性降低可能与肺泡表面活性物质减少引起的肺泡表面张力增高和肺不张、肺水肿导致的肺容积减少有关.ARDS患者血管外肺水(extravaseular lung water,EVLW)显著增多、肺血管通透性增高为其特征性病变[1].本研究监测ARDS患者肺顺应性,同时应用脉搏轮廓连续心输出量(pulse index continuous cardiac output,PiCCO)技术监测血管外肺水指数(extra vascular lung water index,EVLWI),并且对两者进行相关性分析,研究ARDS患者血管外肺水与肺顺应性相关关系,还对影响ARDS患者预后的相关因素进行了分析,现报道如下.
