摘要
目的探讨脑微出血(cerebral microbleeds,CMBS)与白质疏松(leukoaraiosis,LA)的相关性,并对CMBS的好发部位进行研究,从而对脑小血管病有更为深入的认识,并对脑血管病患者的个体化治疗提供更为可靠的依据。方法收集2011年10月~2014年3月在云南省第一人民医院神经内科住院的脑血管病患者112例,行头部核磁共振磁敏感成像及T2加权像检查,确定有LA患者84例为病例组,无LA的患者28例为对照组。根据Aharon-Ptretz的评分法将LA按严重程度分为0~4级共5级。为减少遗漏,采用磁敏感原始图像对所有患者的CMBS进行计数,原始图像分为相位图像和幅值图像。原始图像多达500余张,分层很薄,相位图像和幅值图像各占一半。幅值图像对CMBS的显像较为清晰,本研究在幅值图中对CMBS进行分区域计数。结果有LA组与无LA组的微出血数存在显著性差异,有LA组的CMBS数显著多于无LA组,统计学分析显示了两者严重程度之间的正相关性。另对CMBS的好发部位进行了研究,研究结果显示皮质-皮质下区、基底节和丘脑是CMBS的好发部位,脑干和小脑也可见CMBS。结论同属于脑小血管病的LA严重程度与CMBS计数之间存在正相关性,CMBS在基底节区、额叶、颞叶、丘脑、顶叶、枕叶好发率相对较高,小脑半球、脑干次之,侧脑室旁、半卵圆中心、岛叶少见。
Objective To discuss the correlation between cerebral microbleeds and leukoaraiosis,as well as predilection sites of cerebral microbleeds. In view of the study,we can provide more reliable evidence for individual treatment of our patients with cerebral vascular disease. Methods To collect 112 patients with Cardiovascular disease from October2011 to March 2014 in the Department of neurology of the first people's Hospital of Yunnan province. They were evaluated for the presence of cerebral microbleeds and leukoaraiosis using susceptibility-weighted imaging and T2 weighted imaging examination,84 of them were identified with leukoaraiosis. In the meantime 28 of them without leukoaraiosis were selected as control group. According to the evaluation method of Aharon-Ptretz leukoaraiosis according to severity can be divided into five grades from grade 0 to 4. In order to reduce the error,we counted microbleeds of patients by using susceptibility-weighted imaging as the original image. Over 500 original images were divided into phase and amplitude images. The layer was very thin. Phase and magnitude images were half and half. Magnitude images was more clear to microbleeds,and subregional count on microbleeds in magnitude images was used in our study. The data were analyzed by SPSS13. 0. Results The number of microbleeds between the two groups had significant statistical difference. Microbleeds of groups with leukoaraiosisc were more than groups without leukoaraiosisc,and their severity was positively related. We also had study about predilection site of microbleeds,and it showed ortical and subcortical areas,basal ganglia and thalamus were predilection sites of microbleeds. It also could be seen on brainstem and cerebellum. Conclusion Severity grade of leukoaraiosis and microbleeds counts are positively related. Our study shows ortical and subcortical areas,basal ganglia and thalamus are predilection sites of microbleeds. It also can be seen on brainstem and cerebellum.
