摘要
目的:探讨联合检测血清糖类抗原(CA)15-3、CA125、癌胚抗原(CEA)对乳腺癌的诊断价值。方法对112例乳腺癌患者和49例乳腺良性疾病患者进行血清CA15-3、CA125、CEA联合检测。比较2组患者及乳腺癌组不同临床分期患者3种肿瘤标志物含量和阳性率,并比较3种肿瘤标志物联合与单项检测对乳腺癌的诊断效能。结果乳腺癌组血清CA15-3、CA125及CEA水平和阳性率高于乳腺良性疾病组(P<0.05);乳腺癌Ⅲ~Ⅳ期患者血清CA15-3、CA125及CEA水平和阳性率高于Ⅰ~Ⅱ期患者(P<0.05);乳腺癌患者肿瘤标志物联合检测阳性率高于任何一种单项检测;联合检测提高了Ⅰ~Ⅱ期和Ⅲ~Ⅳ期患者的阳性率,Ⅰ~Ⅱ期患者提高幅度较大;3种肿瘤标志物联合检测提高了乳腺癌患者单项检测的敏感性、准确度和阴性预测值,但特异性和阳性预测值稍降低。结论 CA15-3、CA125、CEA联合检测可弥补单项检测的不足,对于肿瘤的早期诊断、临床分期具有重要意义。
Objective To investigate the clinical value of combined detection of serum carbohydrate antigen ( CA) 15-3,CA125,carcinoembryonic antigen ( CEA) in the diagnosis of breast cancer .Methods Serum CA15-3,CEA,and CA125 were detected in 112 patients with breast cancer and 49 patients with benign breast disease .Content and positive rates of the 3 tumor markers in the 2 groups and different stages of the breast cancer group were compared ,and value of combined detection of the 3 tumor markers and detection of single tumor marker in the diagnosis of breast cancer were compared .Results Serum level and positive rates of CA15-3,CA125 and CEA in the breast cancer group were higher than those of the breast benign disease group ( P〈0.05);serum level and positive rates of CA 15-3,CA125 and CEA of stage Ⅲ~Ⅳbreast cancer were higher than those of the stageⅠ-Ⅱbreast cancer (P〈0.05);combined detection of the 3 tumor markers had higher positive rates in breast cancer pa-tients than that of any single detection;Combined detection improved the positive rates of stageⅠ-Ⅱ and stage Ⅲ-Ⅳ breast cancer patients ,stageⅠ~Ⅱpatients had significant increased;combined detection of the 3 tumor markers in patients with breast cancer improved individual sensitivity ,accuracy and negative predictive value ,while reduced the specificity and positive predictive value slightly.Conclusion Combined detection of CA15-3,CA125,CEA can make up for the shortage of single detection ,and it is important for early diagnosis and clinical staging .
