摘要
目的探讨连续性血液净化治疗妊娠急性脂肪肝的临床疗效。方法回顾性分析2006年1月～2013年12月在我院诊治的64例妊娠急性脂肪肝患者,行常规治疗同时采取连续性血液净化治疗的34例孕妇为治疗组,30例采用终止妊娠、常规保肝抗炎治疗孕妇为对照组,观察两组孕妇治疗后转氨酶、血胆红素、凝血功能、血糖等指标的变化以及死亡率。结果治疗组34例患者的转氨酶、血胆红素、凝血功能、血糖等指标与治疗前比较有显著改善,对照组30例患者的转氨酶、血胆红素、凝血功能、血糖等指标与治疗前比较有所改善,但改善程度不及观察组,两组上述观察指标比较,差异有统计学意义（P〈0.05）。治疗组34例患者全部治愈,死亡率为0;对照组30例患者中4例死亡,死亡率为13.3%。治疗组患者的死亡率明显低于对照组患者,差异有统计学意义（P〈0.05）。结论连续性血液净化治疗妊娠急性脂肪肝可提高临床疗效,降低孕妇死亡率,值得临床推广应用。
Objective To study the clinical effect of continuous blood purification in the treatment of acute fatty liver of pregnancy. Methods 64 patients with acute fatty liver of pregnancy, who were retrospectively analyzed in our hospital from January 2006 to December 2013,were divided into treatment group of 34 cases who were treated with continuous blood purification treatment based on routine treatment,and control group of 30 cases who were treated with termination of pregnancy,conventional protecting liver and anti-inflammatory.To observe the indexes change of transaminase,hemobilirubin,coagulation function,blood glucose and etc,and the mortality after treatment between the two groups. Results To observe the indexes between the two groups,the indexes of transaminase,hemobilirubin,coagulation function,blood glucose and etc before and post treatment has significantly improved in treatment group of 34 cases and some improvement in control group of 30 cases,the improvement degree in control group were lower than which in treatment group,the differences was statically significant（P〈0.05）.The mortality in treatment group was 0 with all of the 34 patients cured,while which was 13.3% in control group with 4 patients dead,the mortality of patients in treatment group was lower than which in control group,the differences was statically significant（P〈0.05）.Conclusion Continuous blood purification in the treatment of acute fatty liver of pregnancy could improve the clinical efficacy and reduce the maternal mortality,is worthy of clinical application.
