摘要
目的上肢康复机器人联合传统康复治疗技术在脑卒中后偏瘫患者肩关节半脱位中的应用研究。方法选取本院神经内科卒中单元和康复医学科门诊及住院的60例脑卒中后偏瘫肩关节半脱位患者,将其随机分为试验组和对照组,每组各30例。对照组患者进行传统康复治疗,试验组患者在对照组基础上给予上肢康复机器人训练,连续治疗2周后观察两组患者的临床疗效、Fugl-Meye运动功能评分(FMA)、功能独立性评定量表(FIM)、肩关节半脱位评分(AHI)及肩关节徒手肌力测定(MMT)的评分变化情况。结果试验组治疗总有效为93.33%,对照组为76.67%,两组比较差异具有显著性(P<0.05);试验组患者治疗后FMA、FIM、MMT评分均高于治疗前,差异显著(P<0.05),AHI评分较治疗前降低,且与对照组治疗后比较差异显著(P<0.05)。结论上肢康复机器人训练能够提高脑卒中后偏瘫肩关节半脱位的临床疗效,提高患者上肢运动功能、独立生活能力和肩关节肌力,同时减少肩关节半脱位的发生,值得临床推广应用。
Objective To observe and analyze the clinical rehabilltation of upper limb robot in the treatment of shoulder subluxation of stroke patients with hemiplegia. Method According to the random number table, 60 cases shoulder subluxation of stroke patients with hemiplegia in our hospital were randomly divided into control group and experimental group, 30 cases for each. Control group was taken with routine rehabilitation treatment, while experimental group added upper limb robot on the basis of control group. The clinical recovery, FMA-UE, FIM, AHI and MMT score of the two groups were compared after 2 weeks treatment. Result The total effective rate was 76.67%in control group, higher than that in experimental group （93.33%）, there was statistical difference between the two groups （P〈0.05）;there was statistical signiifcant difference in the experimental group before and after treatment （P〈0.05）;upper limb robot group was signiifcantly better than control group in reducing AHI score and increase FMA, FIM, MMT score, there was statistical signiifcance between the two groups （P 〈 0.05）. Conclusion Upper limb robot can relieve the clinical symptoms of shoulder subluxation of stroke patients with hemiplegia.
