多频听觉稳态诱发反应在突聋听力检查中的应用及其价值 被引量:3

摘要 目的探讨多频听觉稳态诱发反应(ASSR)在突聋听力检查中的应用及其价值。方法选择2010年8月至2012年6月在长航总医院耳鼻咽喉科就诊并符合中华耳鼻咽喉科学会制订的突聋诊断依据的双耳突聋患者20例,上述患者在同一天内的安静或自然睡眠状态下分别进行ASSR、听性脑干反应(ABR)和纯音测听检查,分析ASSR与纯音测听和ABR的相关性。结果 0.5、1.0、2.0、4.0 k Hz ASSR与纯音测听相关性分别为0.834、0.921、0.961、0.935。其中0.5 k Hz ASSR阈值与纯音测音阈值的差值在10-20 d B,其余频率差值在5-15 d B。除0.5 k Hz外,其他几个频率段ASSR与纯音测听结果均有较高的相关性(P〈0.05)。ASSR与Click ABR的比较表明两种测试方法所得结果具有较高的相关性(P〈0.05)。结论 ASSR频率特异性好,刺激强度高,能同时测试左右耳共8个频率的听阈,是一种有较高临床应用价值的客观测听方法。ASSR与纯音测听和ABR有极高的相关性,在突聋诊断和听力损失程度评定中有其应用价值。 Objective To discuss on the application and value of multiple auditory steady-state responses(ASSR) in audio acuity test for sudden deafness.Methods Twenty patients who were diagnosed as binaural sudden deaf according to the diagnostic criteria made by Chinese Society of Otorhinolaryngology of the Chinese Medical Association in Yangtze River Shipping General Hospital from Aug.2010 to Jun.2012 were tested with pure tone audiometry test,ASSR and Click auditory brainstem response(Click ABR),when they were quiet or sleeping naturally in same day.The correlation of ASSR with ABR and pure tone audiometry test was analyzed.Results Comparison of ASSR and pure tone audiometry test thresholds at 0.5,1.0,2.0,4.0 k Hz were 0.834,0.921,0.961 and 0.935 respectively(P〈0.05).At 0.5 k Hz,the difference was 10-20 d B,while in other carrier frequencies,the differences were 5-15 d B.Results of ASSR and Click ABR showed high correlation with each other,which indicated ASSR could evaluate the function of the hearing as well as Click ABR(P〈0.05).Conclusion ASSR has good frequency-specificity,with high stimulus intensity,it can obtain 8 frequencies at both ears simultaneously.Therefore,it is a promising objective audiometry method.ASSR is highly correlated with pure tone audiometry test and ABR,and has great value in the diagnosis and hearing loss evaluation of sudden deafness.
