摘要
目的 评价依达拉奉对老年患者人工髋关节置换术后血清脂联素(adiponectin,ADP)、S-100β蛋白及术后认知功能的影响.方法 选择90例65~93岁接受人工髋关节置换术患者,采用随机双盲均分为依达拉奉组(E组)和对照组(C组).E组于术前30 min静脉注射依达拉奉60 mg(溶于100ml生理盐水,输注速度200m l/h),C组给予等量生理盐水.分别于手术前24 h及术后6、24、48、72 h用简易精神状态检查量表(MMSE)评估患者的认知功能,并采集空腹静脉血标本测定血清ADP及S-100β蛋白浓度.结果 术后6~48 h,E组MMSE评分明显高于C组(P<0.05).与术前24 h比较,术后6~72 h两组血清S-100β蛋白浓度明显升高(P<0.05),血清ADP浓度明显降低(P<0.05).术后6~72 h,E组血清S-100β蛋白浓度明显低于C组(P<0.05),血清ADP浓度明显高于C组(P<0.05).结论 依达拉奉减轻老年患者人工髋关节置换术后早期认知功能损害及降低术后血清S-100β蛋白浓度可能与上调血清ADP水平有关.
Objective To evaluate the effect of edaravone on serum the adiponectin (AMP), S- 100β and postoperative cognitive function in elderly patient after artificial hip replacement. Methods Ninety ASA Ⅰ or Ⅱpatients, aged 65-93 years, scheduled for artificial hip replacement, were randomized into edaravone group (group E, n=45) and control group (group C, n=45). In group E edaravone 60 mg was intravenous injected preoperative 30 rain (dissolved in 100 ml saline in- fusion, rate 200 ml/h), in group C, received same volume of saline. Cognitive function was assessed by Mini-Mental State Examination (MMSE), at 24 h before operation and 6, 24,48 and 72 h after op- eration, and venous blood samples were collected for determination of serum ADP and S-100p proptein levels. Results MMSE scores in group E were higher than in group C at postoperative 6-48 h (P〈 0.05). Compared with preoperative 24 h, the serum levels of S-100β protein in two groups were increased significantly, whereas ADP levels were decreased significantly at postoperative 6-72 h (P〈 0.05). At postoperative 6-72 h, the serum levels of Sd00p protein in group E were lower, ADP levels were higher than in group C (P〈0.05). Conclusion Edaravone could decrease early postoperative cognitive dysfunction, and declined the serum levels of proptein S-100β through increased the serum AmP levels in elderly patient after artificial hip replacement.
