摘要
目的:探讨低管电压、低浓度对比剂双低扫描技术在颈部动脉成像中的应用。方法:60例患者随机分为A、B两组。A组32例,采用管电压120KV,对比剂,欧乃派克350mg I/ml;B组28例,采用管电压100KV,对比剂,威视派克270mg I/ml。由两名高年资放射科医师对图像质量评价,测量并计算图像噪声、信噪比、对比噪声比,记录CTDIvol和DLP,计算有效辐射剂量。采用SPSS18.0统计软件,分析两组患者一般资料(BMI、扫描范围)、图像质量、碘用量及辐射剂量有无统计学意义。结果:A组与B组患者一般资料、对比剂用量差异无统计学意义(P>0.05);A组与B组图像质量评分差异无统计学意义(P>0.05);A组与B组碘总量、ED、CTDIvol、DLP差异有统计学意义(P<0.05),颈动脉分叉处血管CT值、SNR、CNR差异有统计学意义(P<0.05),噪声差异无统计学意义(P>0.05)。结论:应用双低扫描模式可获得较好的图像质量,可满足临床诊断需求,同时大幅度降低患者所受辐射剂量及碘总量。
Objective: To explore the image quality and the radiation dose in the carotid computed tomographic angiography by using low dose scanning technology and low concentrative contrast medium . Methods: One-hundred patients were randomly selected into two groups(group A and group B). 32 cases of A group (control group) were examined with 120 kV , Omnipaque (350mgI/ml ) contrast medium,28 cases of group B(experimental group) were examined with 100 kV, Visipaque(270mgI/ml) contrast medium.Two observers double-blinded independently assessed the image quality score and measured the different image parameters including image noise, signal-to-noise ratio and contrast-to-noise ratio,recorded the CTDIvol and DLP and accounedt effective radiation. SPSS18.0 statistics software was applied. The patients general information, image quality, the amount of iodine and radiation dose were compared between the two groups. Results: Patients of A and B groups have no statistical significance between general information,the amount of contrast medium(P&gt;0.05).There was no statistical significance with image quality between group A and B groups(P&lt;0.0167), but have statistically significance with total iodine intake, ED, CTDIvol and DLP. The difference between group A and group B was statistically significant with Carotid bifurcation vascular CT value, SNR, CNR(P&lt;0.05), the noise was no significant difference(P&gt;0.05). Conclusions: Patients scanned with dobule low scan mode can obtain better image quality and image quality is satisfactory for diagnosis while obviously reducing patient radiation dose and the total iodine .
