摘要
目的观察糖皮质激素两种用药途径治疗突发性耳聋耳鸣近期疗效的比较。方法将60例突发性耳聋耳鸣的患者随机分为治疗组(30例,30耳)和对照组(30例,30耳),每组在应用扩张血管、营养神经治疗药物的基础上,治疗组加用鼓室注射甲泼尼龙琥珀酸钠注射液,对照组加用静脉滴注甲泼尼龙琥珀酸钠注射液,两组疗程均为10 d,随访6个月。结果治疗组患者耳鸣痊愈5例,显效7例,有效8例,总有效率为66.7%。对照组患者耳鸣痊愈2例,显效3例,有效6例,总有效率36.7%。两组有效率的差异有统计学意义(χ2=4.34,Ρ<0.05)。治疗过程中无中耳炎、鼓膜穿孔、听力进一步下降等并发症发生。结论局部注射治疗突发性耳聋耳鸣的的疗效优于全身用药。
Objective To investigated the recent efficacy of systemic compared with local steroid in the treatment of tinnitus in patients with Sudden Hearing Loss.Methods 60 patients were randomized divided into two groups.Based on the conventional therapy such as blood vessel expander,never nutritional agents,30 cases（30 ears）in control group were treated by intratympanic methylprednisolone in-jection,30 cases（30 ears） in treatment group were treated by intravenous methylprednisolone injection,effect in two group compared. The total course was 10 days and all patients were followed up for half a year.Results In treatment group,5 patients experienced com-plete recovery and 7 a marked recovery,8 a slight recovery,and 10 no recovery.The general effective rate was 66.7%.In control group, 2 patients reached complete recovery,3 a marked recovery,6 a slight recovery,and 19 no recovery.The general effective rate was 36.7%.Treatment group had statistically significant difference compared with control group with regard to the general effective rate （χ^2 =4.34,P〈0.05）.No unexpected side effects such as otitis media,performed tympanic membrane and hearing loss worsened oc-curred during the injection and follow up period.Conclusion The treatment for tinnitus in patients with sudden deafness by local ster-oid injection is more effective.
