峰流速仪联合儿童哮喘控制测试在社区儿童哮喘管理中的应用评价 被引量:7

摘要 目的评价峰流速仪联合儿童哮喘控制测试(childhood asthma control test,C-ACT)在社区儿童支气管哮喘患者管理中的应用效果。方法将门诊就诊的106例哮喘患儿随机分为试验组54例和对照组52例,试验组发放简易峰流速仪监测呼气峰流速及复诊时进行C-ACT,两组均观察1年,每1-3个月复诊1次,比较试验组与对照组哮喘控制水平、长期控制药物使用依从性。结果两组随诊1年后哮喘控制、部分控制及未控制差异有显著性,试验组临床控制率为70.4%,对照组为19.2%。两组随诊1年后长期控制药物使用依从性试验组达79.6%,而对照组为50.0%,其中随诊1、3个月两组差异无显著性(P〉0.05),而6、9、12个月差异有显著性(P〈0.01)。医生评估哮喘控制分级与患者C-ACT评估哮喘控制分级一致性好,Kappa值为0.769。结论峰流速仪联合C-ACT更能准确、全面地反映病情,适用于社区医务人员及患者本人对病情的评估,且可提高患者用药依从性,对于哮喘患者的管理具有较好的指导作用。 Objective To evaluate the application value of the peak expiratory flow combined with childhood asthma control test ( C-ACT) in community management children who has bronchial asthma. Method 106 cases of outpa-tient children with asthma were randomly divided into 54 cases of the experimental group and 52 cases of comparison group, children in the experimental group are given a simple peak expiratory flow to monitor the peak expiratory flow and asked to perform Asthma Control Test during referrals, the two groups were both observed for 1 year, referral every 1-3 months once time, asthma control level and the compliance of long-term control medication are compared between the experimental group and the comparison group. Result Followed up for one year, the difference of asth-ma control, portion control and non-control between the two groups was statistically significant, clinical control rate of experimental group was 70. 4%,but the comparison group was only 19. 2%. At the same time, compliance of the long-term control medication of the experimental group is 79. 6%, while the comparison group is 50. 0%. Followed up for one month or three months,the difference between the two groups was not statistically significant (P>0. 05), and followed up for 6,9, 12 months, difference between the two groups was statistically significant (P<0. 01). The consistency of doctors' assessment of asthma control grading with C-ACT grading of patients is good, the Kappa val-ue is 0. 769. Conclusion The peak expiratory flow combining the asthma control test can reflect the patients' condi-tion more accurately and comprehensively, when used it widely to assess the patients' condition by community health workers and patients themselves, compliance of patients will improve and guidance for the management of asthma patients will be better.
