摘要
目的:分析纳洛酮辅助无创正压通气治疗对慢性阻塞性肺疾病(COPD)并发呼吸衰竭患者血气指标及血清学指标的影响。方法:选择在本院接受治疗的COPD并发呼吸衰竭患者116例作为研究对象,随机分为观察组(58例)接受纳洛酮辅助无创正压通气治疗,对照组(58例)接受单纯无创正压通气治疗。比较两组的血气指标、血清炎症因子水平、血清预后相关因子水平等差异。结果:(1)观察组治疗后动脉血PaO2、PH水平高于对照组,PaCO2水平低于对照组,差异均具有统计学意义(P<0.05);(2)观察组治疗后血清白介素-13(IL-13)、白介素-18(IL-18)、可溶性细胞间黏附分子-1(sICAM-1)、前列腺素E2(PGE2)、高敏C反应蛋白(hs-CRP)等因子水平均明显低于对照组,差异均具有统计学意义(P<0.05);(3)观察组治疗后血清α1抗胰蛋白酶(α1-AT)、D二聚体(D-Dimer)、脑钠肽(BNP)水平低于对照组,甲状腺激素(FT3)水平高于对照组,差异均具有统计学意义(P<0.05)。结论:纳洛酮辅助无创正压通气治疗有助于改善COPD并发呼吸衰竭患者的通气及氧合水平,降低全身炎症反应,优化预后相关指标。
Objective: To analyze effect of Naloxone combining with noninvasive positive pressure ventilation therapy on blood gas indexes and serum indexes of COPD patients complicated with respiratory failure. Methods: 116 cases of COPD com- plicated with respiratory failure patients treated in our hospital were enrolled and randomly divided into observation group (58 cases) who received Naloxone combining with noninvasive positive pressure ventilation therapy, and control group (58 cases) who received plain noninvasive positive pressure ventilation therapy. Then differences of blood gas indexes, serum inflammato ry factor levels and serum prognosis-related factor levels of both groups were compared. Results: 1) After treatment, artery brood PaO2 and PH level of observation group were higher than those of control group PaCO2 level was lower than that of control group (P〈0.05); 2) After treatment, serum factor levels of IL-13, IL-18, sICAM-1, PGE2 and hs-CRP, etc of ob-servation group were all significantly lower than those of control group (P〈0.05);3) After treatment, serum α1-AT, D-Dimer and BNP levels of observation group were lower than those of control group; FT3 level was higher than that of control group (P〈0.05). Conclusion: Naloxone combining with noninvasive positive pressure ventilation therapy helps to improve ventilation and oxygenation levels of COPD complicated with respiratory failure patients, reduce systemic inflammatory response and optimize prognosis-related indexes.
