肺泡表面活性物质辅助机械通气治疗新生儿呼吸窘迫综合征的疗效及对血液指标的影响 被引量:29

摘要 目的:研究肺泡表面活性物质辅助机械通气治疗新生儿呼吸窘迫综合征的疗效及对血液指标的影响。方法:将2013年5月~2014年9月期间我院收治的80例新生儿呼吸窘迫综合征纳入研究,随机分为两组,观察组接受肺泡表面活性物质辅助机械通气治疗,对照组仅接受机械通气治疗,比较两组患者的呼吸功能指标、血清细胞因子含量以及肺泡灌洗液中信号通路功能。结果:两组经治疗后结果显示:观察组患者的吸气阻力(RAW)值低于对照组,氧合指数(PaO2/FiO2)、肺动态顺应性(CD)值高于对照组(P〈0.05);观察组患者的血清Ⅱ型肺泡细胞表面抗原(krebs von den lungen-6,KL-6)、巨噬细胞移动抑制因子-1(MIF-1)、高迁移率族蛋白1(HMGB-1)含量均低于对照组(P〈0.05);观察组患者肺泡灌洗液中Gq/11、1,4,5-三磷酸肌醇(IP3)、甘油二酯(DAG)、表皮生长因子受体(EGFR)、丝裂原活化蛋白激酶(MAPK)、p38的mRNA含量均低于对照组(P〈0.05)。结论:肺泡表面活性物质辅助机械通气治疗有助于改善呼吸功能、缓解炎症反应、抑制Gq/11信号通路和EGFR信号通路的激活,是治疗新生儿呼吸窘迫综合征的理想方法。 Objective: To study the effect of pulmonary surfactant assisted mechanical ventilation treatment on blood in- dex of neonatal respiratory distress syndrome. Methods.. A total of 80 cases of neonatal respiratory distress syndrome in our hospital from 2013 May to 2014 September were enrolled and randomly divided into two groups. Patients in observation group received pulmonary surfactant assisted mechanical ventilation treatment, while control group received mechanical ventilation treatment. Then respiratory function index, serum cytokine content and signal pathway function in BALF were compared. Re- suits.. Results of two group after treatment showed that, RAW value of observation group were significantly lower than that of control groupl PaO2/FiO2 , CD value were significantly higher than those of control group (P〈0.05) ; serum KL-6, MIF-1, HMGB-1 content of observation group were significantly lower than those of control group (P〈0.05 ) ; Gq/11, IP3, DAG, EGFR, MAPK, p38 mRNA contents in BALF of observation group were significantly lower than those of control group (P〈0.05). Qonclusions.. Pulmonary surfactant assisted mechanical ventilation treatment is helpful to improve respiratory function, relieve inflammation response, inhibit the activation of Gq/11 signaling pathway and EGFR signaling pathway; it's an ideal method in treating neonatal respiratory distress syndrome.
