2型糖尿病患者促甲状腺激素水平与胰岛素抵抗的相关性研究 被引量:15

The relationship of thyroid stimulating hormone and insulin resistance in patients with type 2 diabetes mellitus

摘要 目的探讨T2DM患者促甲状腺激素水平(TSH)与IR的关系。方法选取213例T2DM(T2DM)患者及107名正常对照(NC)者。以HbA1c 7%为切点再将T2DM组分为高HbA1c亚组和低HbA1c亚组,分析TSH与IR的相关性。结果相关分析结果显示,TSH与胰岛素抵抗指数(HOMA-IR)呈正相关(r=0.302、0.495,P<0.05)。亚组分析结果显示,两亚组TSH与HOMA-IR呈正相关(r=0.311、0.584,P<0.05)。多元回归分析显示,TSH和BMI是IR的独立危险因素。结论 T2DM患者TSH水平与HOMA-IR呈正相关。 Objective To assess the relationship between thyroid stimulating hormone (TSH ) levels and insulin resistance (IR) in patients with type 2 diabetes mellitus (T2DM ). Methods 213 cases of T2DM and 107 normal controls (NC) were collected for this retrospective study. Based on HbA1 c cut‐off of 7% ,diabetic patients were divided into two subgroups :HbA1 c≥ 7% ,and HbA1 c〈 7% . Linear regression analyses were used to evaluate the association of TSH and HOMA‐IR. Results Linear regression analysis showed that TSH levels were positively associated with HOMA‐IR both in T2DM and NC groups (r=0.302 ,P=0.002 ;r=0.495 ,P〈0.001 ,respectively ). Subgroup analysis related the same linear relationship in HbA1c≥ 7% subgroup (r= 0.584 ,P〈 0.001) and HbA1c〈 7% subgroup (r=0.311 ,P = 0.038 ). Conclusion TSH levels within reference range were linearly and positively associated with HOMA‐IR in type 2 diabetic patients and non‐diabetic subjects ,especially in patients with poor glucose control (HbA1 c≥7% ).
