尿路感染粪肠球菌与屎肠球菌的耐药性分析 被引量:26

Drug resistance of Enterococcus faecalis and Enterococcus feces causing urinary tract infections

摘要 目的探讨医院尿路感染患者中段尿中粪肠球菌及屎肠球菌的耐药性,为临床治疗粪肠球菌及屎肠球菌尿路感染提供依据。方法选取2010年3月-2013年3月收治的500例医院尿路感染患者,共分离出粪肠球菌126株及屎肠球菌85株,分析粪肠球菌、屎肠球菌的耐药性。结果粪肠球菌对四环素、红霉素、高浓度庆大霉素、高浓度链霉素的耐药率分别为77.78%、67.46%、37.30%、32.54%,对替考拉宁、万古霉素、替加环素的耐药率均为0;屎肠球菌对红霉素、青霉素G、氨苄西林、环丙沙星、莫西沙星等抗菌药物的耐药率>90.0%,对替考拉宁、万古霉素、替加环素的耐药率均为0;126株粪肠球菌中产酶6株,产酶率为4.76%;85株屎肠球菌中产酶11株,产酶率为12.94%,两组比较差异有统计学意义(P<0.05)。结论粪肠球菌及屎肠球菌对抗菌药物的耐药性具有较大差异,多药耐药肠球菌的出现也为临床的治疗带来很大难度;对于肠球菌属感染患者的治疗需要重视对菌株的培养以及耐药性分析,减少抗菌药物的盲目使用,以降低耐药菌株的产生,提高治疗效果。 OBJECTIVE To investigate the drug resistance of Enterococcus f aecalis and Enterococcus f eces isolated from midstream urine specimens of the patients with urinary tract infections so as to provide guidance for clinical treatment of the urinary tract infections caused by the E .f aecalis and E .f eces .METHODS A total of 500 patients with urinary tract infections who were treated in the hospital from Mar 2010 to Mar 2013 were enrolled in the study ,totally 126 strains of E .f aecalis and 85 strains of E .feces were isolated ,and the drug resistance of the E . f aecalis and E .f eces was analyzed .RESULTS The drug resistance rates of the E .f aecalis to tetracycline ,erythro‐mycin ,high concentration gentamicin ,and high concentration streptomycin were 77 .78% ,67 .46% ,37 .30% , and 32 .54% ,respectively ;the drug resistance rates to teicoplanin ,vancomycin ,and tigecycline were 0 .The drug resistance rates of the E .feces to erythromycin ,penicillin G ,ampicillin ,ciprofloxacin ,and moxifloxacin were more than 90 .0% ;the drug resistance rates to teicoplanin ,vancomycin ,and tigecycline were 0 .Of the 126 strains of E .f aecalis ,6 strains produced enzymes ,with the enzyme‐producing rate of 4 .76% ,and 11 of 85 E .feces strains produced enzymes ,with the enzyme‐producing rate of 12 .94% ,there was significant difference between the two groups (P〈0 .05) .CONCLUSIONS The E .f aecalis and E .feces vary significantly in the drug resistance , and the emergence of multidrug‐resistant Enterococci strains has challenged the clinical treatment .It is necessary to pay great attention to the culture of strains and analysis of drug resistance for the patients with Enterococcus spp infections and decrease the abuse of antibiotics so as to reduce the drug‐resistant strains and improve the therapeu‐tic effect .
