摘要
目的探讨超声介导对脑肿瘤切除手术的应用价值。方法将70例经CT/MRI诊断为颅内肿瘤的患者,按随机数表法分为实验组(35例)和对照组(35例),两组在显微外科手术基础上术前均结合头颅影像资料确定皮瓣和骨窗范围;实验组术前颞窗和枕窗常规超声探查,术中将探头置于硬脑膜或脑表面实时探查病灶,在手术进程中监测以指导术者对病灶的切除;对照组术者仅根据术前CT/MRI检查结果进行手术操作;术后观察记录两组病灶残余率并定期对患者进行随访,对两组术后Karnofsky功能状态评分进行评价。结果实验组术中肿瘤病灶定位准确率达100%,实验组术后肿瘤残余率2.86%,对照组肿瘤残余率14.28%,组间比较有显著差异(P<0.05);两组患者术后KPS评分比较,也具有显著性差异,实验组显著优于对照组(P<0.05)。结论术中超声介导能准确对颅内肿瘤病灶进行定位,实现对病灶切除的实时监测,有助于指导术者准确彻底切除肿瘤。
Objective To explore the impacts of Ultrasound Mediated Technique on cere- bral tumor resection. Methods Divide 70 cases of cerebral tumor patients (diagnosed with CT/ MRI) at random into a control group (35) and an experimental group (35). Determine the scope of the skin flap and the bone window in the both groups based on the microsurgery and head im- age. As to the experimental group, conduct the preoperative exam for the anterior temporal win- dow and occipital window by the conventional transducer, and put the transducer in dura or on the surface of the brain for real--time nidus exploration,monitoring and instructing the nidusl resec- tion at surgery. For the control group, conduct the operation just based on the preoperative CT/ MRI. Observe the residual rate of the nidi,make the regular follow--up for patients and access the Karnofsky function for the both groups. Results For the experimental group, the tumor nidus po- sitioning accuracy and residual rate are 100% and 2. 86% respectively, while for the control group,the latter number is 14. 28%. The numbers prove the significant difference between the groups(P〈0. 05). The significant difference also exists in the postoperative KPS assessment, with the experimental group better than the control one(P〈0. 05). Conclusion Ultrasound Me- diated Technique is applicable for accurate cerebral tumor nidus positioning and nidus resection real--time monitoring so as to provide instruction to the operators at surgery. Therefore it is of great clinical significance in improving the operation security and effects.
