摘要
目的比较芬吗通(17-β雌二醇片/17-β雌二醇地屈孕酮片)与人工周期治疗异常子宫出血(AUB)的临床疗效。方法选取2013年2月至2014年3月于本院南院妇产科门诊就诊的120例AUB患者,其中月经过多17例、月经过频21例、月经过少13例、月经稀发16例、月经不规则21例、经期长18例、围排卵期出血14例。随机分为观察组(芬吗通组,60例)和对照组(人工周期组,60例)。采用治疗后两组的正常月经周期持续时间来评价疗效,比较两组的疗效和不良反应。结果治疗后,观察组60例,痊愈35例,有效16例,无效9例,有效率为85.0%;对照组60例,痊愈32例,有效17例,无效11例,有效率为81.7%。两组疗效比较,差异无统计学意义(P>0.05)。但两组月经过多、月经过频、月经过少、月经不规则、经期长、围排卵期出血的治疗痊愈率和有效率比较,差异均无统计学意义(P均>0.05)。两组各种不良反应相比较,差异无统计学意义(P>0.05)。结论芬吗通治疗AUB疗效确切,对月经过频、月经稀发、经期长及围排卵期出血均有一定疗效,而且服用方便,无明显副作用,可以达到与人工周期相同的疗效。
Objective：To compare the clinical efficacy of Fenmoston（17βestradiol tablets/17βestradiol and dydrogesterone tablets）and artificial cycle in treating abnormal uterine bleeding（AUB）.Methods：A total of 120 AUB patients including 17 patients with menorrhagia,21 epimenorrhea,13hypomenorrhea,16 oligomenorrhea,21 irregular menstruation,18 long menstrual period,14 periovulatory bleeding were recruited in Department of Obstetrics and Gynecology,South Branch of Shanghai East Hospital Affiliated to Tongji University from February 2013 to March 2014.The patients were randomly divided into treatment group（n=60）and control group（n=60）.The patients in treatment group took orally 1 white pills（1 mg 17βestradiol）daily for 14 days,and then 1gray pills（1 mg 17βestradiol and 10 mg dydrogesterone tablet）daily for another 14 days.The patients in control group took orally 1 mg Progynova daily from the 5^th day of the menstrual cycle for 21 days,and added 8 mg medroxyprogesterone acetate tablet daily from the 11^th day for 10 days,and then withdrawal 3-7days until the next menses.The patients in both groups were treated continuously for three cycles.The efficacy and side effect were compared between the two groups.Results：Thirty five patients were cured,16 effective,9invalid,and the effective rate was 85.0%in the treatment group.Thirty two patients were cured,17 effective,11invalid,the effective rate was 81.7%inthe control group.There was no significant difference in curative effect between the two groups（P〈0.05）.There were no significant differences in the treatment efficiency of menorrhagia,hypomenorrhea,irregular menstruation epimenorrhea,oligomenorrhea,long menstrual period and periovulatory bleeding among the both groups（P〉0.05）.Conclusions： Administration of Fenmoston in treating abnormal uterine bleeding has definite clinical effect,particularly for epimenorrhea,oligomenorrhea,long menstrual period and periovulatory bleeding.It is convenient,without obvious side effects,and can achieve the similar effect as the artificial cycle.
