摘要
目的探讨体外过敏原与潮气呼吸肺功能联合测定对儿童喘息性疾病的鉴别诊断价值。方法选取支气管哮喘患儿54例(A组)、毛细支气管炎患儿60例(B组)、喘息性肺炎患儿63例(C组)为研究对象。比较各组患儿吸入过敏原阳性率、食入过敏原阳性率、嗜酸性粒细胞阳离子蛋白(ECP)、总IgE(TIgE)、每公斤体质量潮气量(VT)、达到呼气峰流速时呼出容积与呼气容积比值(VPTEF/VE)、达到呼气峰流速时间与呼气时间比值(TPTEF/TE)。结果 A组、B组吸入过敏原阳性率、食入过敏原阳性率、TIgE、ECP、VT均显著高于C组(P<0.05);A组、B组VPTEF/VE、TPTEF/TE均显著低于C组(P<0.05)。A组TIgE及ECP水平显著高于B组(P<0.05);A组吸入过敏原阳性率、食入过敏原阳性率、VT、VPTEF/VE、TPTEF/TE与B组比较差异无统计学意义(P<0.05)。结论通过体外过敏原与潮气呼吸肺功能检测,可了解喘息性疾病患儿气道炎症及肺部阻塞情况,有利于进行鉴别诊断。
Objective To evaluate the differential diagnosis value of allergen test in vitro combine with tidal breath- ing lung function in children with asthmatic diseases. Methods 54 patients with bronchial asthma (group A), 60 patients with capillary bronchitis (group B), 63 patients with asthmatic pneumonia (group C) were included in the present study. The inhaled allergens positive rate, food allergens positive rate, eosinophil cationic protein (ECP), total IgE (TIgE), tidal volume per kilogram( VT), the ratio of tidal volume exhaled at PTEF to total tidal volume (VPTEF/VE), and the ratio of exhaled time at peak tidal expiratory flow to total expiratory time (TPTEF/TE) were compared among the three groups. Results The inhaled allergens positive rate, food allergens positive rate, TIgE, ECP and VT of A group and B group were significantly higher than those of C group(P〈0.05). The VPTEF/VE and TPTEF/TE of A group and B group were significantly lower than those of C group(P〈0.05). The TIgE and ECP of A group were significantly higher than those of B group(P〈0.05). The inhaled allergens positive rate, food allergens positive rate, VT, VPTEF/VE and TPTEF/TE of A group was no statistically significant difference compared with B group (P〈0.05). Conclusion Using allergen test in vitro combine with tidal breathing lung function can help to understand the children with asthmatic disea- ses airway inflammation and lung congestion, conducive to carry out the differential diagnosis.
