摘要
目的应用速度向量成像技术(VVI)评价孕母甲状腺功能减退胎儿右心室心肌力学变化。方法应用VVI,采集并分析64例母亲妊娠期甲状腺功能减退的胎儿(观察组)及125例正常胎儿(对照组)的标准四腔心切面动态声像图,比较两组右心室心肌长轴上收缩期峰值速度(Vs)、收缩期峰值应变(S)、收缩期峰值应变率(SRs)及舒张期峰值速度(Vd)、舒张期峰值应变率(SRd)。结果观察组及对照组右心室心肌长轴Vs、Vd随孕周的增加而增大(F=7.87~38.03,P〈0.05);S、SRs、SRd与孕周无明显相关(P〉0.05)。观察组妊娠28周后右心室心肌长轴Vd、32周后Vs、24周后S、SRs、SRd均低于对照组(t=2.65~3.49,P〈0.05)。观察组右心室整体Vs、Vd、S、SRs、SRd均低于对照组,差异有显著性(t=2.16~3.65,P〈0.05)。结论孕母甲状腺功能减退胎儿右心室长轴收缩及舒张功能降低;VVI可以客观、准确地反映胎儿心脏功能的变化。
Objective To assess the changes of myocardiac mechanics of right ventricle in fetus of mother with hypothy- roidism during pregnancy using velocity vector imaging (VVI). Methods Standard dynamic four-chamber view sonograms were collected and analyzed using VVI in 64 fetuses of mothers with hypothyroidism during pregnancy (observation group) and 125 healthy subjects (control group). The myocardium systolic maximum velocity (Vs), diastolic maximum velocity (Va), systolic maximum strain (S), systolic maximum strain rate (SRs), and diastolic maximum strain rate (SRd) of the right ventricle were compared between the two groups. Results Vs and Vd enlarged along with the increase of gestational weeks (F=7.87-38.03, P〈0.05), whereas S, SR~ and SRa were not (P〉0.05). In the observation group, Vs after 32 weeks, Vd after 28 weeks, and S, SRs and SRa after 24 weeks in right ventricle were lower than that in the control group (t =2.65--3.49,P〈0.05). Conclusion The systolic and diastolic function of right ventricle reduce in fetus of mother with hypothyroidism during pregnancy. Velocity vec- tor imagine can obiectivelv and accurately reflect the changes of fetal heart function.
