摘要
目的探讨"术前谈话"情景教学模式在产科学临床教学中的应用价值。方法选择在某医药学院2012级五年制临床医学专业120名实习学生为研究对象,将其随机分为两组,对照组60人,实验组60人,每6名学生为一个小组,分别进行常规教学模式训练和"术前谈话"情景模拟教学训练。结果 (1)两组匿名问卷调查结果表明实验组学生对教学形式的满意度和学习知识的主动性、积极性均高于对照组学生,差异具有统计学意义(P<0.05)。在提高与患者的交流能力、沟通的自信心方面以及临床综合思维培养方面与对照组相比,两组差异明显(P<0.01)。(2)两组SEGUE问卷调查结果显示实验组学生的医患沟通能力明显高于传统教学的对照组学生,差异具有统计学意义(P<0.01)。(3)两组学生OSCE成绩比较,两组学生在病史采集方面都能按老师要求系统询问,分数和合格率比较,差异无统计学意义(P>0.05),但在仪表态度、专业知识、临床思维、书面告知表达方面,实验组学生成绩优于对照组学生,两组差异具有统计学意义(P<0.05)。(4)两组出科考核成绩分析显示,基础知识测试中,两组比较差异无统计学意义(P>0.05),而在临床操作技能考核中,两组差异具有统计学意义(P<0.05)。结论 "术前谈话"情景模拟教学训练是一种实际可行的解决医患矛盾、建立和谐医患关系的具体培养方式,值得推广。
Objectives To investigate the application value of scene teaching mode of preoperative conversation in the clinical teaching process of obstetrics department. Methods 120 intern students who majored in clinical medicine science of 2012 five-year class were selected from Hubei Medicine College as the objects of this study. They were randomly divided into two groups, the experimental group and control group with 60 students in each group, in which every 6 students were taken as a small group, then conducted conventional pattern of teaching practice and scene simulation teaching training of "preoperative conversation" respectively. Results (1) Two groups of anonymous questionnaire survey results showed that the experimental group students' satisfaction in the form of teaching and learning initiative and enthusiasm of knowledge students were higher than control group, and the difference had statistical significance (P〈0. 05). In improving confidence aspect of communication with the patient's ability to communicate, and to cultivate the students' clinical comprehensive thinking, compared with control group, the differences between the two groups were significant (P〈0.01). (2)Two groups SEGUE questionnaire survey from the study results showed that the ability of doctor-patient communication was significantly higher than the traditional teaching students of control group, difference had statistical significance (P〈0. 01). (3) Compared the OSCE score of students in the two groups, as well as medical history acquisition according to the requirements of the teacher included system inquiries, scores and qualified rate, there was no statistically significant difference (P〉0. 05), but in the instrument attitude, professional knowledge, expressed in clinical thinking and notify, students in the experimental group were better than the control group, and the differences between the two groups had statistical significance (P〈0.05). (4) An analysis on the final examination results showed that there was no statistical significance in basic knowledge test between the two groups (P〉0. 05), while there were statistical differences in clinical skills examination (P〈0. 05). Conclusions Scene simulation teaching training of "preoperative conversation" is a kind of practical, feasible and specific mode to solve doctor-patient contradiction and establish harmonious doctor-patient relationship, which is worth promoting.
