摘要
目的探讨早期护理干预对腰椎间盘突出症术后神经根粘连的预防作用。方法统计分析2012年3月至2014年3月西安医学院第二附属医院收治的78例腰椎间盘突出症患者的临床资料。结果术后2周,3、6、12个月研究组患者日本骨科学会评分均显著高于对照组,差异有统计学意义(P<0.05);研究组患者康复优良率89.7%(35/39)显著高于对照组66.7%(26/39),差异有统计学意义(P<0.05);研究组残留腰痛、腿痛发生率5.1%(2/39)、7.7%(3/39)均显著低于对照组的15.4%(6/39)、12.8%(5/39),差异有统计学意义(P<0.05);术后研究组患者抑郁自评量表评分显著低于对照组,差异有统计学意义(P<0.05);术后6个月研究组患者直腿抬高试验阳性率7.7%(3/39)显著低于对照组43.6%(17/39),差异有统计学意义(P<0.05)。结论早期护理干预能够有效预防腰椎间盘突出症术后神经根粘连,促进患者早日康复。
Objective To investigate the preventive effect of early nursing intervention on postoperative nerve root adhesions after lumbar disc herniation operation .Methods The clinical data in 78 cases of lumbar disc herniation in our hospital from March 2012 to March 2014 were statistically analyzed .Results The Japanese Orthopedic Assoc‐ition (JOA) scores in postoperative 2 weeks ,6 ,12 months in the study group were significantly higher than those in the control group with statistical difference(P〈0 .05);the rehabilitation excellent rate in the study group was 89 .7%(35/39) ,which was significantly higher than 66 .7% (26/39) in the control group with statistical difference(P〈0 .05) ,the incidence rates of residual lumbar pain and leg pain in the study group were 5 .1% (2/39) and 7 .7% (3/39) ,which were significantly lower than 15 .4% (6/39) and 12 .8% (5/39) in the control group ,the differences were statistically significant (P〈 0 .05) ,the postoperative self‐rating depression scale(SDS) scores in the study group were significantly lower than those in the control group with statistical difference(P〈 0 .05);the positive rate of straight leg raising test after 6 months in the study group was 7 .7% (3/39) ,which was significantly lower than 43 .6% (17/39) in the control group ,the difference was statistically significant (P〈0 .05) .Conclusion The early nursing intervention can effectively prevent postoperative nerve root adhesions after lumbar disc herniation operation and promote early recovery of patients .
