摘要
目的探讨家庭医生签约式服务模式对社区高血压患者管理的效果。方法选取上海市闸北区天目西街道社区卫生服务中心2013年10月开始采取家庭医生签约式服务模式管理的高血压患者405例为干预组,同时选取高血压患者276例为对照组,至2014年9月对比管理前后两组患者血压水平、BMI指数及空腹血糖和4项脂代谢指标的测量情况。结果与对照组比较,实施家庭医生签约式服务模式管理的干预组患者的血压、BMI指数控制及空腹血糖和4项脂代谢指标结果均明显优于对照组。经统计学检验分析:收缩压、三酰甘油、高密度脂蛋白、低密度脂蛋白四项指标前后差异有极显著性意义(P<0.01);舒张压、空腹血糖、总胆固醇及BMI四项指标前后差异有显著性意义(P<0.05)。结论对社区高血压患者实施家庭医生签约式服务模式后,患者血压及高血压相关危险因素指标均得到较好控制,在社区推行家庭医生签约式服务管理模式能更好提升社区高血压管理的效果。
OBJECTIVE To explore family doctor contracted service model for community management of patients with hypertension re- sults. METHODS Select Zhabei District Tianmu West Street Community Health Center as the object. Since October 2013, it took the family doctor contracted service model management for 405 cases of hypertensive patients as the intervention group. While 276 cases of hypertensive patients selected for the control group. Until September 2014, measuring blood pressure levels before and after con- trast administration groups of patients, BMI index and fasting plasma glucose and lipid metabolism of 4. RESULTS Compared with the control group, Blood pressure in the intervention group were signed to implement family doctor managed service model, BMI index control, fasting glucose and lipid metabolism 4 results were significantly better than the control group. According to statisti- cal test analysis, systolic blood pressure, triglycerides, high density lipoprotein, low-density lipoprotein, these four indicators between before and after was huge significantly different（P〈0.01）. Diastolic blood pressure, fasting glucose, total cholesterol and BMI index, these four indicators between before and after was significantly different（P〈0.05）.CONCLUSION After the implementa- tion of hypertension patients in community family physicians contracted service model, blood pressure and hypertension in patients with associated risk factors indicators all have better control. Implementation of family doctors contracted-service management model to better enhance the effectiveness of community management of hypertension in the community.
