摘要
目的探讨重度骨质疏松性胸腰椎压缩骨折患者行球囊扩张椎体后成形术(PKP)的分型治疗方案的治疗效果。方法选择2011年1月至2014年5月就诊的胸腰椎骨质疏松性压缩骨折患者160例为研究对象,随机分为研究组和对照组,每组各80例。研究组将患者分为假关节型、进展型、空洞型和单纯型,分别采用不同的PKP治疗方案,对照组患者行PKP手术治疗,但不进行PKP分型。术后对所有患者的治疗效果进行评价,采用SF-36健康调查表和VAS表对患者术后健康状况和疼痛视觉模拟进行评分。观察术后骨水泥渗漏情况。结果术后研究组患者的有效率为83.75%,对照组患者的有效率为61.25%,研究组的有效率明显高于对照组;手术后研究组患者的VAS评分和SF-36评分均明显优于对照组患者,且研究组伤椎凸角恢复和椎前中柱均高度恢复较对照组好;术后研究组患者的并发症总发生率为29%,对照组患者的并发症总发生率为40%,研究组患者的并发症发生率明显小于对照组患者,差异均有统计学意义(P<0.05)。结论对胸腰椎骨质疏松性压缩骨折患者进行分型,采取不同的方案治疗,能够提高临床疗效,改善患者的术后VAS评分和SF-36评分,降低术后并发症发生率,值得临床推广。
Objective To investigate the effect of classified percutaneous kyphoplasty(PKP)treatment for severe osteoporotic thoraco-lumbar compression fractures. Methods One hundred sixty cases of severe osteoporotic thoracolumbar compression fractures treated in our hospital from January 2011 to May 2014 were selected. The patients were divided into study group(80 cases)and control group(80 cases)randomly. Study group were further divided into false joint type,progressive type,hole type and simple type,and different PKP treatment options were adopt-ed according to the type of fracture;the control group underwent PKP,but without typing. Postoperative evaluation was performed. The health sta-tus and visual analog pain of all patients were scored with SF - 36 health questionnaire and VAS scale after operation. The bone cement leakage of postoperative patients was observed. Results The PKP efficiency was higher in study group(83. 75% )than control group(61. 25% ). The VAS score and SF - 36 score of the study group were significantly better than the control group,and the vertebral injury recovery and anterior vertebral column height in study group were better than control group;the overall complication rate of the study group(29% )was less than control group (40% )( P 〈 0. 05). Conclusion Differential PKP treatment performed according to the types of osteoporotic vertebral compression fracture can improve the clinical efficacy,postoperative VAS score and SF - 36 score,and reduce the incidence of postoperative complications.
